JCI标准下2015—2017年常熟市第一人民医院麻醉药品和第一类精神药品应用分析

摘　　要：目的:了解国际联合委员会(joint commission international,JCI)标准下常熟市第一人民医院(以下简称"我院")麻醉药品和第一类精神药品使用情况,为临床合理用药提供参考。方法:从我院信息管理系统中提取2015—2017年麻醉药品和第一类精神药品的相关数据,依据世界卫生组织日剂量分析法,对使用量、销售金额、用药频度(defined daily dose system,DDDs)等进行统计分析。结果:2015—2017年,我院麻醉药品和第一类精神药品的销售金额分别为750 775.0、879 511.0和1 236 108.0元,2016、2017年分别比上一年增长了17.15%和40.54%;我院麻醉药品和第一类精神药品的DDDs分别为32 795、31 410和36 339,在JCI认证后,2017年比2016年增长了15.69%。3年来,销售金额排序居前3位的药品为枸橼酸舒芬太尼注射液、盐酸羟考酮缓释片(10 mg)和芬太尼透皮贴剂;DDDs排序居前3位的药品为芬太尼透皮贴剂、盐酸羟考酮缓释片(10 mg)和枸橼酸芬太尼注射液。3年来,口服+外用制剂的销售金额和DDDs累计构成比分别为63.24%(1 812 832.3/2 866 394.0元)和68.24%(68 606/100 544)。结论:在JCI标准下,我院麻醉药品和第一类精神药品的使用更趋合理,但仍需严格管控,使之更加安全、有效、经济地用于临床。OBJECTIVE： To investigate the application of narcotic drugs and the first class of psychotropic drugs under the JCI standard in Changshu No. 1 People＇s Hospital（ hereinafter referred to as ＂our hospital＂）,so as to provide references for clinical rational drug application. METHODS： Related data of narcotic drugs and the first class of psychotropic drugs during 2015-2017 were extracted from HIS and analyzed in terms of usage amount,consumption sum and defined daily dose system（ DDDs） according to DDD analysis recommended by WHO. RESULTS： During 2015-2017,the consumption sum of narcotic drugs and the first class of psychotropic drugs were respectively 750 775. 0 yuan,879 511. 0 yuan and 1 236 108. 0 yuan,with growth rates of 17. 15% in 2016 and 40. 54% in 2017 respectively; the DDDs of narcotic drugs and the first class of psychotropic drugs were respectively 32 795,31 410 and 36 339,after JCI authentication,the DDDs in 2017 had increased by 15. 69% than that of 2016. In the past three years,the top 3 drugs ranked in consumption sum were respectively sufentanil citrate injection,Oxycodone hydrochloride sustained tablets（ 10 mg） and fentanyl transdermal patches; the top 3 drug ranked in DDDs were respectively fentanyl transdermal patches,Oxycodone hydrochloride sustained tablets（ 10 mg） and sufentanil citrate injection. In the past three years,the accumulative constituent ratio of consumption sum and DDDs of oral and external preparation were respectively 63. 24%（ 1 812 832. 3/2 866 394. 0 yuan） and 68. 24%（ 68 606/100 544）. CONCLUSIONS： Under JCI standard,the application of narcotic drugs and the first class of psychotropic drugs is basically rational,which still needs further control and monitoring to ensure safe,effective and economical application.
