摘 要:目的:探讨临床药师干预Ⅰ类切口手术围术期预防性应用抗菌药物的效果。方法:回顾性选取2017年11月至2018年4月某二级甲等医院收治的Ⅰ类切口手术患者547例,其中干预前(2017年11月至2018年1月未经过"抗菌药物整治活动")283例,干预后(2018年2—4月通过设立临床药师参与诊疗工作以及加强合理用药管理和控制)264例,提取患者电子病历信息,比较干预前后抗菌药物预防性使用率、抗菌药物品种选择合理率、给药时间合理率、用药疗程合理率。结果:干预后,Ⅰ类切口手术围术期抗菌药物预防性使用率由干预前的70.32%降至28.79%,抗菌药物品种选择合理率由干预前的57.79%升至75.00%,给药时间合理率由干预前的19.60%升至61.84%,用药疗程合理率由干预前的38.69%升至55.26%。结论:临床药师的干预可以降低Ⅰ类切口手术围术期抗菌药物预防性使用率,提高抗菌药物品种选择、给药时间及用药疗程的合理性,使医院抗菌药物的使用更加规范、抗菌药物的管理更加科学,值得推广。OBJECTIVE: To probe into the effects of preventive application of antibiotics in type Ⅰ incision in perioperative period before and after intervention by clinical pharmacists. METHODS: 547 patients with type Ⅰ incision admitted into a secondary class A hospital from Nov. 2017 to Jan. 2018 were retrospectively extracted,among which 283 cases were before intervention( without "remediation activity"from Nov. 2017 to Jan. 2018) and 264 cases were after intervention( through establishment of clinical pharmacists' participation in the diagnosis and treatment work and strength of rational application of drug management and control from Feb. to Apr. 2018). The information of patients' electronic medical records were extracted,and the preventive application rate of antibiotics,rational selection rate of antibiotics,rational tare of administration time and rational rate of treatment course were compared before and after intervention by clinical pharmacists. RESULTS: After intervention,the preventive application rate of antibiotics in type Ⅰ incision in perioperative period decreased from 70. 32% to 28. 79%,the rational selection rate of antibiotics increased from 57. 79% to75. 00%,the rational tare of administration time increased from 19. 60% to 61. 84%,and the rational rate of treatment course increased from 38. 69% to 55. 26%. CONCLUSIONS: The intervention of clinical pharmacists can reduce the preventive application of antibiotics in type Ⅰ incision in perioperative period,improve the rational selection rate of antibiotics,rational tare of administration time and rational rate of treatment course,so that the application of antibiotics in hospitals is more standardized and the management of antibacterial drugs is more effective. It is worth of being generalized.
