临床药师对肺腺癌并发肾功能不全患者培美曲塞个体化用药的监护

摘　　要：目的:探讨临床药师在肺腺癌并发肾功能不全患者使用培美曲塞化疗过程中的作用,为合理用药提供参考。方法:回顾性分析临床药师对1例肺腺癌并发肾功能不全患者使用培美曲塞化疗时的药学监护过程。结果:临床药师通过分析培美曲塞药动学、药效学特点,结合患者肾功能水平,计算该患者合适的培美曲塞个体化给药剂量,并对化疗方案进行全面监护。结论:临床实践中,临床药师可通过文献证据,从药动学、药效学的角度,协助医师合理使用化疗药。OBJECTIVE： To probe into the role of clinical pharmacists in individualized medication of pemetrexed for patient with lung adenocarcinoma complicated with chronic kidney disease,so as to provide reference for the rational drug application. METHODS： Retrospective analysis was conducted on the process of pharmaceutical care participated by clinical pharmacists for one patient with lung adenocarcinoma complicated with chronic kidney disease.RESULTS： Through analyzing the pharmacokinetics and pharmacodynamics characteristics of pemetrexed,combined with information about the patients＇ renal function,the clinical pharmacists calculated the proper individualized drug dosage for patients and provided comprehensive supervision for the chemotherapy regimens. CONCLUSIONS： In clinical practice,clinical pharmacists can assist physicians in the rational application of the chemotherapeutic drugs from the perspective of pharmacokinetics and pharmacodynamics.
