手足挛三针为主治疗中风后痉挛性瘫痪的临床随机对照研究

摘　　要：目的：采用多中心、大样本、前瞻性临床随机对照研究观察靳三针疗法治疗中风后痉挛性瘫痪的临床疗效。方法：收集中风后痉挛性瘫痪患者166例,随机分为治疗组（颞三针＋手足挛三针）82例,对照组（颞三针＋手足三针）84例,在治疗前及治疗4周后使用改良Ashworth痉挛量表（MAS）进行疗效评定,用临床痉挛指数（CSI）量表、临床神经功能缺损程度评分（NDS）及日常生活活动能力（Barthel）指数进行功能评估并进行统计分析处理,观察靳三针疗法在治疗中风后痉挛痉挛性瘫痪中的最佳穴位配伍。结果：治疗组总有效率为94.9%,对照组总有效率为90.2%,两组总有效率相比较,差异有统计学意义（P〈0.05）。两组患者治疗后各项功能评估评分与同组治疗前相比差异均有明显统计学意义（P〈0.01）。结论：靳三针疗法治疗中风后痉挛性瘫痪效果显著,且以颞三针＋手足挛三针效果为优,值得推广应用。Objective：A multi-centered,large sample,prospective randomized controlled clinical trial was designed to observe the clinical curative effect of spastic paralysis after stroke by Jin＇s 3-needle therapy. Methods：We collected 166 cases of spastic paralysis in acute stage of stroke and randomly divided them into treatment group（ Nie 3-needle therapy ＋ hand-foot Luan 3-needle therapy,82 cases）,and control group（ Nie 3-needle therapy ＋ hand-foot Luan 3-needle therapy,84 cases）. We observed the modified Ashworth spasticity scale（ MAS）,clinical spasticity index（ CSI） scale,clinical neurological impairment score（NDS）,limb motor function score,simplified Fugl-Meyer and ADL index（Barthel） before and after treatment. After evaluated and statistically analyzed the results,we observed the best acupoints combination of Jin＇ s 3-needle therapy in the treatment of spastic paralysis after stroke. Results：The total effective rate of the treatment group was 94. 9%,while that of the control group was90. 2%,and the difference was statistically significant（ P〈0.05）. The difference of the score of functional evaluation of two groups before and after treatment was statistically significant（ P〈0.01）. Conclusion：The Jin＇s 3-needle therapy,especially Nie3-needle therapy ＋ hand-foot Luan 3-needle therapy,has significant effect in treatment of spastic paralysis in acute stage of stroke. The therapy is worthy of popularization and application.
