摘  要:目的建立SD大鼠急性创伤模型,探讨冻干人血小板(Freezing-dried Platelets,FDP)对SD大鼠急性创伤的促愈合作用。方法 SD大鼠60只,在每只大鼠背部脊柱两侧各剪下2个直径为1.2 cm的圆形全层皮肤。将每只大鼠背部4个创面分配到4个组:FDP组、新鲜血小板(platelet rich plasma,PRP)组、重组型人表皮生长因子(Recombinant human epidermal growth factor,rh EGF)组和空白对照组(blank control,BC)。分别于造模后即刻给予FDP(0.85 m L/cm2)、PRP(0.85 m L/cm2)、rh EGF(0.5 m L/cm2)和凡士林纱布进行治疗,每间隔两天清洗伤口后更换敷料。于d5、d7、d10、d14肉眼观察、拍照,并记录创面愈合情况,计算创面愈合率,取各时间点创缘新生皮肤组织进行HE染色、Masson三色染色和CD34免疫组化三项病理学实验,并进行相关统计分析。结果肉眼观察各组大鼠创面均随治疗时间延长而趋于缩小,4个重点观测点得出的综合创面愈合率排序为PRP〉FDP〉rh EGF〉BC组。治疗d5的创伤愈合率相近,没有显著差异;d7,PRP组、FDP和rh EGF组的愈合率均高于BC组,差异有统计学意义(P〈0.05),前三组组间差异没有统计学意义(P〉0.05);d10时,前3组的愈合率均高于BC组,差异有统计学意义(P〈0.05);组间比较,PRP组创面愈合率高于FDP和rh EGF组(P〈0.05)。d14,组间的创面愈合率没有明显差异。3种组织细胞学观察炎性细胞浸润、胶原纤维生成、血管生成增生和再上皮化等病理结果显示:FDP与PRP同样能缩短创伤愈合炎症期、促进胶原纤维的生成、加快血管生成、促进肉芽组织生长,从而促进急性创伤组织的修复作用。结论 FDP具有与新鲜血小板相近的促进SD大鼠急性创面愈合的作用,并具有优于单一生长因子(rh EGF)的促愈合效果。Objective To study the effect of frozen dry platelet( FDP) on wound healing of by SD Rats acute wound models. Methods Two full-thickness skin wounds created on both said of spine in 60 SD rats. Four wounds were classified into 4 groups: FDP group,fresh platelet rich plasma( PRP) group,recombinant human epidermal growth factor( rh EGF)group and blank control( BC) group. These wounds were treated with FDP( 0. 85 m L/cm2)/PRP( 0. 85 m L/cm2)/rh EGF( 0. 5 m L/cm2) and petrolatum gauze immediately after operation. All dressing were changed every two days after cleaning the wounds. Observe and photographed wound healing,were calculated to wound healing rate. Wound-edge skin were taken at5 th,7 th,10 thand 14 thday to analysis differences of wound healing among four groups by HE staining,Masson Staining and CD34 Immunohistochemistry. Results All of group wound healing with the duration of treatment,the comprehensive rate was PRP FDP rh EGFBC group. At 5 thdays,there was no significant different between the four groups( P〈0. 05); at 7 th days,the wound healing was significantly accelerated by treated with FDP,PRP and rh EGF compared with control group( P〈0. 05),no significantly difference among three group( P〈0. 05); at 10 thdays,the wound healing of FDP,PRP and rh EGF group was significantly accelerated compared with control group( P〈0. 05),the group PRP was higher than group FDP and rh EGF( P〈0. 05); at 14 thdays,there was no significant different between the four groups( P〈0. 05). Histologically,in FDP and PRP group,inflammation was shortened,wounds showed higher velocity of angiogenesis and more granulation tissue,formation and mature of collagen fibers. Conclusion This study provides evidence that topical FDP can not only accelerate the velocity and promote the wound healing but also likely to produce the similar effect to PRP,and better than rh EGF.
