经脐单孔腹腔镜在子宫肌瘤剔除术中的应用研究现状及进展

摘　　要：子宫肌瘤手术在妇科领域已非常成熟,随着技术的发展及外科医师对手术技巧的追求,子宫肌瘤剔除术的术式在不断变化。"no scar"的设想一经提出便受到广泛关注。在技术发展的支持下,单孔腹腔镜技术应运而生,脐部是人体天然瘢痕,经脐单孔腹腔镜技术(laparoendoscopic single-site surgery,LESS)将微创手术的美容效果发挥至极致。LESS在子宫肌瘤剔除术中的应用存在着"筷子效应"、"操作三角"、缝合困难等一些问题,而在不断实践和探索中,随着特殊器械的发明,操作技巧的总结,LESS下的子宫肌瘤剔除术已经可以实现。随着技术的成熟,相信LESS将成为子宫肌瘤剔除术的主要术式。Uterine fibroid surgery is very mature in gynecology, and with the development of technology and the pursuit of surgeons, the surgical methods are constantly changing. Since the idea of ＂no scar＂ proposed, it has raised attention. With the support of technological development, laparoendoscopic single site surgery （LESS） was proposed. The navel is a natural scar in the body, transumbilical LESS brings the perfect effect of minimally invasive surgery to the extreme. Although LESS has some problems in the surgery such as ＂chopsticks-effect＂, ＂operational triangle＂ and difficulty in suturing, while with the constant practice and exploration, the invention of special devices, the sum up of operating skills, myomectomy under LESS has achieved. It＇s believed that with the development of technology, LESS will become the main way for myomectomy.
