摘 要:目的调查金黄色葡萄球菌细胞外基质结合蛋白表达基因ebh的分布及对细菌耐药性的影响。方法随机收集医院2016年12月-2017年7月住院患者非重复标本中分离鉴定的金黄色葡萄球菌共84株。采用头孢西丁纸片扩散法及聚合酶链式反应(PCR)法检测mecA基因以筛选耐甲氧西林金黄色葡萄球菌(MRSA)株;选取MRSA与甲氧西林敏感金黄色葡萄球菌(MSSA)各24株,再通过PCR法结合基因测序对金黄色葡萄球菌携带ebh基因情况进行分析;运用脉冲场凝胶电泳(PFGE)法对其阳性株进行基因分型;采用纸片琼脂扩散法药敏试验分析ebh阳性及阴性金黄色葡萄球菌对抗菌药物的耐药性。结果金黄色葡萄球菌ebh基因总检出率为39.6%(19/48);其中MRSA的ebh基因阳性率为45.8%(11/24),MSSA阳性率为33.3%(8/24),MRSA和非MRSA阳性率比较差异无统计学意义(P=0.083);ebh阳性株PFGE分型为3型,以A型为主,13株占68.4%;ebh阳性MRSA与MSSA株对庆大霉素、克林霉素、红霉素和复方新诺明抗菌药物的耐药率高于ebh阴性的金黄色葡萄球菌,差异均无统计学意义。结论 ebh基因可能是导致金黄色葡萄球菌具有持续感染能力的关键因子之一,其可能影响金黄色葡萄球菌对抗菌药物的耐药性。OBJECTIVE To investigate the prevalence of extracellular matrix binding protein ebhin Staphylococcus aureus and its effect on antibiotic resistance.METHODS A total of 84 nonduplicate clinical S.aureus isolates were collected form Gansu Provincial Hospital in Dec.2016 to Jul.2017.The MRAS strains were detected by methicillin disc agar diffusion and PCR amplification of mecA gene,then the distribution of ebh gene in MRSA and MSSA strains(24 each)was analyzed by PCR and gene sequencing.The ebh-positive strains were genotyped with pulsedfield gel electrophoresis(PFGE).The antibiotic resistance of ebh-postive and negative strains was studied by disc agar diffusion drug sensitivity test.RESULTS The total detection rate of ebh gene in S.aureus isolated from clinical specimens was 39.6%(19/48).The detection rates of ebh gene in MRSA and MSSA were 45.8%(11/24)and33.3%(8/24),respectively.The PFGE of 19 ebh-positive strains displayed 3 kinds of genotypes,which was dominated by A genotype,with 13 strains accounting for 68.4%.The ebh-positive MRSA and MSSA strains showed higher rates of antibiotics resistance to gentamicin,clindamycin,erythromycin and chemitrim as compared to the ebh-negative S.aureus,but the difference was not statistically significant.CONCLUSIONebh gene may be one of the potential virulence factors of Staphylococcus aureus causing sustained infection,and the presence of ebh gene could be related to its antibiotic resistance.
