摘　　要：目的采用立体心电图仪通过Wilson和Frank双导联体系同步采集心电图和心电向量图,及时发现和诊断陈旧性下后侧壁心肌梗死合并左前分支阻滞。方法选择临床诊断明确、有典型陈旧性下后侧壁心肌梗死合并左前分支阻滞特点的心电向量图进行分析。观察心电向量图的改变。结果三例陈旧性下后侧壁心肌梗死合并左前分支阻滞患者的心电向量图特点均符合心电向量图的诊断标准。结论心电向量图对陈旧性下后侧壁心肌梗死合并左前分支阻滞具有较高的诊断价值。Objective To timely identify and diagnose old inferior posterolateral myocardial infarction complicating left anterior fascicular block on synchronously recorded electrocardiogram（ ECG） and vectorcardiogram（ VCG）by the Wilson Frank dual lead system with three-dimensional electrocardiography.Methods We selected and analyzed the VCGs with typical features of definitely diagnosed old inferior posterolateral myocardial infarction complicating left anterior fascicular block.The changes of VCGs were observed.Results The vectorcardiographic features of three cases coincided with vectorcardiographic diagnostic criteria of old inferior posterolateral myocardial infarction complicating left anterior fascicular block.Conclusion VCG has high diagnostic value for old inferior posterolateral myocardial infarction complicating left anterior fascicular block.
