摘　　要：目的分析老年心力衰竭(简称心衰)患者的临床特征。方法选取2016年10月至2017年3月在新乡市中心医院因心衰住院的322例中老年患者。依据世界卫生组织(WHO)年龄分组方法,将年龄≥75岁的133例老年期心衰患者作为老年组,将年龄在45~74岁的189例中年期及老年前期心衰患者作为非老年组,分别从病因类型、左室射血分数(LVEF)、氨基末端B型钠肽原(NT-pro BNP)及血脂等方面对两组患者临床特征进行比较。结果与非老年组心衰患者比较,老年组心衰患者的总胆固醇(TC)水平、甘油三酯(TG)水平、低密度脂蛋白(LDL-C)水平及LVEF水平较低,NT-pro BNP水平较高,差异有统计学意义(P<0.05)。老年组心衰患者冠心病、高血压患病率较高。结论老年心衰患者的血脂水平更低,心功能指标更差,其主要病因为冠心病及高血压,需加强老年心衰患者的二级预防及个体化治疗。Objective To analyze the clinical features in the elderly patients with heart failure（ HF）. Methods Three hundred and twenty-two cases middle-old aged heart failure patients who were hospitalized from October of 2016 to March of 2017 in Xinxiang Central Hospital were selected. According to the World Health Organization（ WHO） age group methods,133 cases who aged were older than 75 years old as the elderly group,and 189 cases who aged were 45 - 74 years old in middle-aged and presenium patients as non-elderly group,the clinical features of the two groups from the cause of disease,left ventricular ejection fraction（ LVEF）,N-terminal pro-B type natriuretic peptide（ NT-pro BNP） and characteristics of blood lipid several aspects were compared. Results Compared with the non-elderly group,total cholesterol（ TC）,glycerin trilaurate（ TG）,low density lipoprotein cholesterol（ LDL-C） and LVEF in the elderly group were lower,NT-pro BNP in the elderly group was higher,the differences were statistically significant（ P〈0. 05）. Etiological factor was coronary heart disease and hypertension rates increased significantly. Conclusion The elderly patients with heart failure have lower blood lipid level and worse cardiac function index,the main heart failure cause is coronary heart disease and hypertension,two level prevention and individualized treatment for elderly patients with heart failure need to be strengthened.
