耳周转移皮瓣修复耳廓皮肤缺损的疗效分析

摘　　要：目的探讨耳周转移皮瓣修复耳廓皮肤缺损的临床疗效。方法回顾性分析2013年1月~2017年6月于我院就诊,因耳廓外伤或肿物切除术致耳廓皮肤缺损病例,分别用游离皮瓣修复术(对照组)与耳周转移皮瓣修复术(观察组)来修复缺损皮肤,比较手术时间、术中出血量、住院时间、总治疗时间、皮瓣坏死发生率等指标,分析两者的差异。结果利用游离皮瓣修复耳廓皮肤缺损在手术时间、术中出血量等方面优于转移皮瓣修复组(P<0.05);但在住院时间、总治疗时间、皮瓣坏死发生率等方面,耳周转移皮瓣修复组却具有更大优势(P<0.05)。结论游离皮瓣修复耳廓皮肤缺损操作相对简单,在没有条件行转移皮瓣修复时,也不失为一个办法;而利用耳周转移皮瓣修复耳廓皮肤缺损,可缩短住院和总治疗时间、减少术后皮瓣坏死发生率,更值得临床推广。OBJECTIVE To investigate the clinical effect of peri-auricular transfer flap on auricular skin defec repairment. METHODS We retrospectively analyzed the patients who suffered from auricle skin defect, due to auricle trauma or tumor resection, in Taizhou Hospital from January 2013 to June 2017. The skin defection was repaired by free flap（control group） or periauricular transfer flap（observation group）. Then we analyzed the differences of operative time、intraoperative bleeding、hospitalization time hospital stays、total treatment time and incidence of skin flap necrosis between the two groups. RESULTS The control group needed less operative time and was followed with less intraoperative bleeding than that of the observation group（P〈0.05）. However, the observation group had more advantages in length of stay, total time of therapy and incidence of skin flap necrosis than those in control group（P〈0.05）. CONCLUSION Using free skin flap to repair the auricle skin defect is relatively simple and could be taken if there is no condition to use transfer the skin flap for repairing.The operation of using periauricular transfer flap to reconstruct cutaneous deficiency of auricle can reduce the use of antibiotics, shorten length of stays and total treatment time and reduce the incidence of skin necrosis. It is worthy of recommendation in clinical using practice.
