摘 要:目的 研究血常规和血脂水平与肺血栓栓塞症(PTE)的相关性,了解血常规和血脂等指标对预测PTE发生的指导意义,进一步明确PTE的发病原因.方法 收集2010年1月至2014年12月在中国医科大学附属盛京医院确诊为PTE的98例患者的血常规和血脂等实验室检查结果,并选取98名排除既往PTE、下肢深静脉血栓、恶性肿瘤、炎症疾病及服用降脂药病史的健康体检者作为对照组,对其血常规和血脂水平等资料进行统计学分析.结果 PTE组总甘油三酯、载脂蛋白B、血白细胞计数均高于对照组(t值分别为-2.502、-2.124、-6.679,P值均<0.05),而高密度胆固醇、载脂蛋白AⅠ、血细胞比容均低于对照组(t值分别为7.108、9.348、4.242,P值均<0.05).单因素及多因素logistic回归分析结果显示,高密度胆固醇(OR =0.194,95%CI:0.057~0.659,P<0.05)、载脂蛋白A Ⅰ(OR =0.044,95% CI:0.009~0.209,P<0.05)、血细胞比容(OR=0.784,95%CI:0.705~0.871,P<0.05)及血白细胞计数(OR=1.737,95%CI:1.312~2.299,P<0.05)与PTE的发生相关.结论 高密度胆固醇、载脂蛋白AⅠ、血细胞比容降低和血白细胞计数升高可能是PTE危险因素.Objective To explore the relationship of routine blood and serum lipid levels with pulmonary thromboembolism (PTE),and evaluate the significance of routine blood and serum lipid in predicting the occurrence of PTE,so as to further clarify the causes of PTE.Methods The levels of routine blood and serum lipid of 98 patients diagnosed as PTE in Shengjing Hospital of China Medical University from January 2010 to December 2014 were reviewed.98 healthy volunteers excluded from PTE or deep vein thrombosis history,without neoplastic or inflammatory diseases and use of lipid-lowering drugs were choosen as controls.The levels of routine blood and serum lipid of two groups were compared.Results The triglyceride,apolipoprotein B,and white blood cell of PTE group were higher than those of control group (t =-2.502,-2.124,-6.679,all P 〈0.05).The high density lipoprotein cholesterol,apolipoprotein A Ⅰ,and hematocrit of PTE group were lower than those of control group (t =7.108,9.348,4.242,all P 〈 0.05).The results showed high density lipoprotein cholesterol (OR =0.194,95%CI:0.057-0.659,P 〈0.05),apolipoprotein AⅠ (OR =0.044,95%CI:0.009 0.209,P 〈0.05),hematocrit (OR =0.784,95% CI:0.705 0.871,P 〈0.05) and white blood cells (OR =1.737,95% CI:1.312-2.299,P〈0.05) were associated with the occurrence of PTE.Conclusions The lower levels of high density lipoprotein cholesterol,apolipoprotein AⅠ,hematocrit and elevated level of white blood cell may be the risk factors of PTE.
