腮腺多形性腺瘤和低度恶性多形性腺瘤的CT表现及初步分析

摘　　要：目的:探讨腮腺多形性腺瘤(PA)及低度恶性PA患者的多层螺旋CT(MSCT)表现及其影像学鉴别诊断价值。方法:回顾性分析经手术病理证实的45例(共48个病灶)PA患者的影像学资料(在术前CT均诊断为良性)。结果:良性病灶33个,低度恶性15个。PA病灶的起源部位、大小(平均最大径:良性组2.10±0.79cm,低度恶性组2.64±1.45cm)、包膜的完整性及强化程度与良恶性之间无明显统计学差异。病灶边界模糊(良性组3/33个,低度恶性组10/15个)、形态不规则(良性组8/33个,低度恶性组9/15个)、密度不均(良性组11/33个,低度恶性组13/15个)及边缘血管样强化(良性组6/33个,低度恶性组10/15个)与病变的良恶性存有显著统计学差异(P<0.05)。结论:MSCT检查有助于腮腺多形性腺瘤的诊断及良恶性的判断,为指导临床治疗及预后提供重要参考价值。Purpose： To investigate the CT findings and imaging diagnosis of parotid pleomorphic adenoma （PA） and low-grade malignant PA. Methods： CT features of 45 cases （48 lesions） parotid PA confirmed by surgery and pathology results were retrospectively analyzed. All these lesions were diagnosed as benign tumors by multi- slice CT before the operation. Results： There were 33 benign lesions and 15 low-grade malignant PA. The features of tumor location, tumor size （average maximum diameter： benign group 2.10 4-0.79cm, low-grade malignant group 2.64＋1.45cm）, integrity of the tumor envelope and strengthen degree in assessment the grade of the tumors were not helpful. Lesions with the fuzzy boundaries （benign group 3/33, low grade group 10/15）, irregular shape （benign group 8/33, low grade group 9/15）, uneven density （benign group 11/33, low grade group 13/15） and vascular like enhancement at the margin of capsule （benign group 6/33, low grade group 10/15） were with statistical significant differences （P〈0.05）. Conclusion： CT examination is helpful for the diagnosis of parotid pleomorphic adenoma and the judgment of its benign or malignancy, and also for the clinical treatment and prognosis estimation.
