摘  要:目的:探讨成人颈动脉分叉部的几何形态与缺血性脑血管疾病的关系。方法:回顾性收集187例同时行颈动脉CT血管造影和头颅MRI检查患者的影像学及临床资料,采用多因素Logistic回归分析,分析可能的相关因素与缺血性脑血管疾病之间的关系。结果:高血压、糖尿病、冠心病、高同型半胱氨酸血症和吸烟史、饮酒史等因素与缺血性脑血管疾病无关(P>0.05)。高血脂、颈动脉分叉部角度与缺血性脑血管疾病有关(P<0.05)。正常人的颈动脉分叉部的几何形态与缺血性脑血管疾病患者明显不同(P<0.05)。缺血性脑血管疾病患者颈动脉分叉角及相邻血管内径均明显大于正常人群(P<0.05)。结论:脑缺血性疾病患者的颈动脉分叉角度更大,颈动脉分叉角度很有可能作为一个预测脑缺血性疾病的有用指标。Purpose: To investigate the relation between carotid bifurcation geometry and ischemic cerebrovascular diseases (ICVD) in adults. Methods: The imaging and clinical data of 187 cases who accepted the examination of carotid artery CT angiography and head MRI at the same time were collected. Logistic multiple factor regression was applied to analyze the relationships between the potential related factors and ischemic cerebrovascular diseases. Results: These factors of hypertension, diabetes, coronary heart disease, hyperhomocysteinemia, smoking history, and drinking alcohol hobby were irrelevant to ICVD (P 〉 0.05). But the factors of hyperlipaemia and the carotid artery bifurcation angle were closely related to ICVD (P〈 0.05). Normal carotid bifurcation geometries are obviously different from those of patients with cerebral ischemia. The carotid artery bifurcation angle and adjacent vascular internal diameter of the patients with cerebral ischemia were significantly greater than those of normal people (P 〈 0.05). Conclusions: The carotid bifurcation angle in patients with cerebral ischemic is even larger. The carotid bifurcation angle is likely to be a useful predictor of cerebral ischemic diseases.
