摘 要:目的探讨重组人干扰素α-2b不同给药方式治疗对手足口病患儿血清细胞因子及免疫功能的影响。方法选择2014年7月至2016年7月收治的120例手足口病患儿作为研究对象,按随机数字表法分为A、B组各60例,两组均采用重组人干扰素α-2b治疗,A组应用肌肉注射方案,B组采用雾化吸入方案,比较两组治疗效果,治疗前、治疗1周均采集患儿外周静脉血,测定血白介素-6(IL-6)、白介素-10(IL-10)、肿瘤坏死因子-α(TNF-α)、乳酸脱氢酶(LDH)等细胞因子水平的变化,同时检测治疗前后患儿免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)水平的变化。结果 A组治疗总有效率低于B组,差异有统计学意义(P<0.05);B组治愈时间、退热时间、皮疹消退时间、口腔溃疡愈合时间均短于A组,差异有统计学意义(P<0.05);治疗5d,两组IL-6、IL-10、TNF-α、LDH水平均降低,IgA、IgM、IgG均上升,与同组治疗前比较差异有统计学意义(P<0.05),但组间比较差异无统计学意义(P>0.05);B组不良反应发生率低于A组,差异有统计学意义(P<0.05)。结论重组人干扰素α-2b雾化吸入治疗手足口病疗效肯定,安全性高,且可改善患儿免疫功能,下调细胞炎症因子水平。Objective To investigate the effect of different administration methods of recombinant human interferon α-2b on serum cytokines and immune function in children with hand-foot-mouth disease (HFMD). Methods A total of 120 children with HFMD treated in our hospital during the period from July 2014 to July 2016 were selected as the subjects, and divided into group A and group B with 60 cases in each group by the random number table method. The two groups were treated with recombinant human interferon α-2b. Group A were treated by intramuscular injection while group B were treated by aerosol inhalation, and therapeutic effects were compared between the two groups. The peripheral venous blood of children was collected before treatment and after 1 week of treatment to determine changes in levels of serum interleukin - 6 ( IL-6 ), interleukin - 10 ( IL- 10 ), tumor necrosis factor α ( TNF- α ) and lactate dehydrogenase ( LDH ). Changes in levels of immunoglobulin A ( IgA ), immunoglobulin G ( IgG ) and immunoglobulin M ( IgM ) in children were detected before and after treatment. Results The total treatment effective rate of group A was lower than that of group B (P 〈 0.05 ). The cure time, fever clearance time, time of rash disappearing and oral ulcer healing time of group B were shorter than those of group A ( P 〈 0.05 ). After 5 days of treatment, levels of IL- 6, IL- 10, TNF- α and LDH were decreased, while levels of IgA, IgM and IgG were increased. Comparedwith those in the same group before treatment, there were statistically significant differences ( P 〈 0.05 ), but there was no significant difference between groups ( P 〉 0.05 ). The incidence of adverse reactions in group B was lower than that in group A (P 〈 0.05 ). Conclusion Aerosol inhalation of recombinant human interferon α- 2b is effective in the treatment of HFMD, with high safety, and can improve the immune function and down regulate levels of cell inflammatory factors in children.
