ACEI联合β-受体阻滞剂治疗高龄高血压合并心力衰竭效果观察

摘　　要：目的:探讨血管紧张素转化酶抑制剂(angiotensin converting enzyme inhibitors,ACEI)联合β-受体阻滞剂治疗高龄高血压合并心力衰竭的疗效。方法:研究对象选取笔者所在医院心内科2015年1月-2017年12月收治高龄原发性高血压合并心力衰竭患者110例,采用随机区组法分为对照组和联合治疗组,每组55例;两组患者均给予托拉塞米治疗,对照组患者加用依那普利治疗,联合治疗组患者则在此基础上与美托洛尔缓释片联用治疗;比较两组患者临床疗效,治疗前后动态血压、心率、超声心动图指标、6 min步行试验指标水平及不良反应发生率等。结果:联合治疗组患者临床疗效显著优于对照组,差异有统计学意义(P<0.05);联合治疗组患者治疗后动态血压、心率、超声心动图指标及6 min步行试验水平均显著优于对照组、治疗前,差异均有统计学意义(P<0.05);两组患者不良反应发生率比较差异无统计学意义(P>0.05)。结论:ACEI与β-受体阻滞剂联用治疗高龄高血压合并心力衰竭疗效明显优于ACEI单用,且未增加不良反应发生风险。Objective：To investigate the clinical effects and safety difference of ACEI used alone and combined with cardiac selective β-receptor blockers in the treatment of elderly patients with essential hypertension and heart failure.Method：110 elderly patients with essential hypertension and heart failure were chosen in the period from January 2015 to December 2017 and randomly divided into two groups including control group（55 patients） with ACEI used alone and combined group（55 patients） with ACEI combined with cardiac selective β-receptor blockers on the basis of torasemide;and the clinical effects,levels of dynamic blood pressure,heart rate,echocardiography index,6 minute walking test index before and after treatment and adverse reaction incidence of both groups were compared.Result：The clinical effects of combined group was significant better than control group（P〈0.05）.The levels of dynamic blood pressure and heart rate of combined group after treatment was significant better than control group and before treatment（P〈0.05）.The levels of echocardiography index and 6 minute walking test index of combined group after treatment was significant better than control group and before treatment（P〈0.05）.There was no significant difference in the adverse reaction incidence between two groups（P〈0.05）.Conclusion：Compared with ACEI used alone,ACEI combined with cardiac selective β-receptor blockers in the treatment of elderly patients with essential hypertension and heart failure can efficiently relieve the symptoms and signs of heart failure,higher blood pressure control effect and heart function,improve the quality of life,and not increase the risk of adverse reactions.
