皮肤牵张器结合克氏针在四肢皮肤软组织缺损中的应用

摘　　要：目的探讨使用皮肤牵张器结合克氏针治疗四肢皮肤软组织缺损的临床疗效。方法 2015年4月—2017年6月收治的38例四肢皮肤软组织缺损患者,创面大小:4cm×3cm^25cm×8cm,合并骨或肌腱外露27例,关节周围损伤21例。先在创面纵轴的皮缘2~3cm处置入直径1.2mm的克氏针,再将皮肤闭合器的卡钩扣于皮内的克氏针上,每日持续牵张闭合创面。结果所有创口均获得闭合。所有患者获随访,随访时间6~18个月,有5例发生钉道处皮肤感染,3例钉道周围皮肤坏死。末次随访时创面闭合良好,无明显瘢痕挛缩,外形美观,无臃肿,弹性良好,触痛觉正常,关节部位功能得到较好保留。结论应用皮肤牵张闭合器结合克氏针是治疗四肢皮肤软组织缺损的可行方法。Objective To investigate the clinical efficacy of skin stretching device combined kirschner wire in repair of the extremity skin soft tissue defects. Methods From April 2015 to June 2017,we treated 38 patients with extremity skin soft tissue defects.The wound sizes were 4 cm×3 cm^25 cm×8 cm.Twenty-seven cases were complicated with bone or tendon exposure,21 cases with peripheral joint injury.Firstly a kirschner wire of 1.2 mm diameter was placed in skin edge with 2~3 cm distance to the longitudinal wound,the hook of the skin closure device was hooked on the skin kirschner wire,the wound-closure was continuously stretched daily. Results All the wounds closure were obtained.All the patients were followed up for 6 to 18 months.There were 5 cases of nail skin infection and 3 cases of skin necrosis.The wound closure was good at the end of the follow-up,no obvious scar contracture,good appearance,no bloat,good elasticity and normal touch,and the joint function retained better. Conclusion It is feasible to treat the soft tissue defects of the extremities with skin stretching device combined with kirschner wire.
