分析两种麻醉方法对老年髋关节置换术的麻醉效果及髋关节功能的影响

摘　　要：目的分析布比卡因腰硬联合麻醉对老年髋关节置换术患者手术麻醉的效果,并分析麻醉对患者术后髋关节功能的影响。方法方便选取该院2017年1—12月收治的164例老年髋关节置换术患者,将其随机分为联合组和对照组,每组82例患者,联合组利用布比卡因腰硬联合麻醉作为手术麻醉方法,对照组则利用传统全身麻醉作为麻醉手段,对比两组麻醉效果差异,术后髋关节功能差异。结果联合组麻醉效果优良率91.46%,对照组79.26%(χ2=4.69),联合组麻醉不良反应6.09%,对照组15.85%(χ2=5.32),两组髋关节功能评分、镇痛评分,阻滞麻醉时间等指标差异有统计学意义(P<0.05)。结论老年髋关节置换术对患者髋关节功能的影响较大,多数患者在术后出现不同程度的髋关功能障碍,利用布比卡因腰硬联合麻醉能够减少患者手术对血流动力学影响,尽可能减少患者术后髋关节功能障碍,值得临床应用推广。Objective To analyze the effect of bupivacaine combined spinal and epidural anesthesia on elderly patients undergoing total hip arthroplasty and analyze the effect of anesthesia on postoperative hip function. Methods Convenient select a total of 164 elderly hip replacement patients admitted to the hospital from January to December 2017 were randomly divided into a combined group and a control group, with 82 patients in each group. The combined group was treated with bupivacaine. Hard and joint anesthesia was used as a surgical anesthetic method. In the control group, traditional general anesthesia was used as an anesthetic method to compare the difference in anesthetic effect between the two groups and postoperative hip function. Results The excellent rate of anesthesia was 91.46% in the combined group and 79.26% in the control group（χ^2=.69）. The adverse reaction was 6.09% in the combined group and 15.85% in the control group（χ^2=.32）.The hip function score, analgesia score, and resistance There were significant differences in other indicators such as anaesthesia time（P〈0.05）. Conclusion The hip arthroplasty for elderly patients has a great influence on hip function. Most patients have different degree of hip dysfunction after operation. The use of bupivacaine combined with lumbar and lumbar anesthesia can reduce the impact of surgery on hemodynamics. As far as possible to reduce postoperative hip dysfunction, it is worth clinical application.
