妊娠中期母体血清NGAL和Cys C水平对子痫前期的预测价值

摘　　要：目的分析妊娠母体26孕周血清中性粒细胞明胶酶相关性脂质运载蛋白(NGAL)和半胱氨酸蛋白酶抑制剂C(Cys C)水平对子痫前期的预测价值。方法回顾性收集该院近三年子痫前期患者60例,同期60例健康妊娠孕妇作为对照组。应用酶联免疫吸附实验(ELISA)法、胶乳增强免疫浊度法和酶法分别检测血清中NGAL、Cys C和血清肌酐(Scr)浓度。应用ROC曲线评估NGAL和Cys C对子痫前期患者早期诊断价值。结果子痫前期组26孕周血清NGAL、Cys C和Scr水平分别为(222.18±40.80)ng/mL、(0.92±0.14)mg/L和(56.03±8.13)μmol/L;正常妊娠组26孕周血清NGAL、Cys C和Scr水平分别为(137.99±24.42)ng/mL、(0.84±0.12)mg/L和(55.50±8.67)μmol/L。与正常妊娠组比较,子痫前期组母体血清NGAL和Cys C平均水平明显升高,差异有统计学意义(P<0.05),血清NGAL水平升高更明显,差异有统计学意义(P<0.05),Scr水平差异无统计学意义(P>0.05)。血清NGAL、Cys C和Scr水平的受试者工作曲线(ROC)曲线下面积分别是0.96、0.69和0.55。结论妊娠中期母体血清NGAL水平可作为子痫前期的预测指标。Objective To analyze the predictive value of serum NGAL and Cystatin C levels for preeclampsia in the 26 gestation weeks.Methods 60 cases of preeclampsia in our hospital in recent three years were retrospectively collected,and 60 normal pregnant women in the same period served as control group.The concentration of serum NGAL,Cystatin C and Scr were detected by ELISA,latex enhanced immunoturbidimetry and enzyme method.ROC curve was used to evaluate the early diagnostic value of NGAL and Cys C in preeclampsia.Results Serum NGAL、Cystatin C and Scr levels were（222.18±40.80）ng/mL,（0.92±0.14）mg/L and（56.03±8.13）μmol/L in preeclampsia of 26 gestation weeks,Serum NGAL、Cystatin C and Scr levels were（137.99±24.42）ng/mL,（0.84±0.12）mg/L and（55.50±8.67）μmol/L in 26 gestation weeks of normal pregnancy group.Compared with the normal pregnancy group,the serum NGAL and Cys C levels in the preeclampsia group increased significantly（P〈0.05）,the serum NGAL level increased more significantly（P〈0.05）,and there was no significant difference in Scr level（P〉0.05）.The area under the ROC curve of serum NGAL,Cys C and Scr levels were 0.96,0.69 and 0.55,respectively.Conclusion The serum NGAL level in the middle trimester of pregnancy can be used as a predictor of preeclampsia.
