右美托咪定对脊柱侧弯矫形术患者全麻诱导和术中唤醒时血流动力学的影响

摘　　要：目的探讨右美托咪定对脊柱侧弯矫正形手术患者全麻诱导和术中唤醒时血流动力学的影响。方法对本院在2015年2月-2017年2月期间收治的60名脊柱侧弯患者的临床资料进行回顾分析,将上述患者分成观察组和对照组,各30人。两组均给予丙泊酚维持麻醉,对照组在此基础上给予生理盐水持续静脉泵注;观察组接受右美托咪定辅助镇静镇痛治疗。综合评价上述两组的全麻诱导期术后唤醒时血流动力学变化和唤醒的时间、唤醒期的出血量等。结果对照组患者的T_0时血压和心率在唤醒之后显著提高;对照组T_2、T_3、T_4时的心率和血压显著高于观察组,且差异存在统计学意义(P<0.05);观察组唤醒时间和出血量较大程度的低于对照组(P<0.05)。结论针对脊柱侧弯手术患者实行右美托咪定嘱咐镇静剂麻醉手段可以缩短术后的唤醒时间,降低唤醒期的出血量,并保证血流动力相对稳定,安全有效,临床效果显著,具有重要的临床推广价值。Objective To investigate the effect of dexmedetomidine on postoperative patients with general anesthesia induced arousal and scoliosis correction during plastic surgery hemodynamics. Methods The clinical data of 60 scoliosis patients treated in our hospital from February 2017 to February 2015 were reviewed and analyzed. The patients were divided into the observation group and the control group, each for 30 patients. The two groups were all given propofol anesthesia, and on this basis, the control group were given continuous intravenous infusion of normal saline, while the observation group were received dexmedetomidine to assist the sedative and analgesic treatment. The hemodynamic changes, the time of wakeup and the amount of bleeding at wake-up period after the induction period of general anesthesia were evaluated comprehensively between two groups. Results Blood pressure and heart rate at T0 in the control group increased significantly after wakeup; Heart rate and blood pressure at T2, T3 T4 in the control group were significantly higher than those in the observation group, and the difference was statistically significant（P〈0.05）; Wake-up time and the amount of bleeding at the observation group were significantly lower than those at the control group（P〈0.05）. Conclusion For patients with scoliosis surgery,dexmedetomidine sedation anesthesia can shorten the wakeup time after operation, reduce the amount of bleeding, and ensure relatively stable hemodynamics. So it is, safe and effective, and it had significant clinical effect and important clinical value.
