超声心动图诊断冠心病节段性室壁运动异常的价值分析

摘　　要：目的:研究并探讨超声心动图诊断冠心病节段性室壁运动异常的临床价值。方法:研究对象选择2015年7月~2016年12月期间在我院就诊且临床诊断为冠心病的50例患者,所有患者均接受超声心动图检查,观察其超声心动图,分析是否出现节段性室壁运动异常。以冠状动脉造影检查结果为参照,计算超声心动图对冠心病患者节段性室壁运动异常的诊断灵敏度、特异度、准确性、阳性预测值、阴性预测值,再采用Kappa一致性检验,分析超声心动图诊断结果与冠脉造影诊断结果之间的一致性。结果:50例冠心病患者中,经冠脉造影检查,有37例确诊为冠状动脉狭窄率75%以上并节段性室壁运动异常,其余13例冠状动脉狭窄狭窄率65%以下,未发现明显节段性室壁运动异常。经计算,超声心动图对节段性室壁运动异常的诊断灵敏度为97.30%,准确性为98.00%,阴性预测值为82.86%,特异度、阳性预测值均达到100.00%。经一致性检验,超声心动图诊断结果与冠脉造影诊断结果之间的一致性达到良好(Kappa=0.782)。结论:超声心动图可对冠心病患者存在的节段性室壁运动异常予以准确检查,其诊断价值良好。Objective： To study and explore the clinical value of echocardiography in the diagnosis of segmental ventricular wall movement in coronary heart disease. Methods： a total of 50 patients who were diagnosed as coronary heart disease in our hospital from July 2015 to December ~ 2016 were selected. All patients underwent echocardiography,and their echocardiography was observed to analyze whether there was segmental wall motion abnormality. The results of coronary angiography as the reference,the calculation of echocardiography in patients with coronary heart disease and segmental wall motion abnormalities of the diagnostic sensitivity,specificity,accuracy,positive predictive value,negative predictive value,then the Kappa consistency test,the consistency between the analysis results of echocardiography and coronary angiography in the diagnosis of the. Results： in 50 patients with coronary heart disease,37 cases were diagnosed as coronary artery stenosis by coronary angiography. The rate of coronary artery stenosis was more than 75% and segmental wall motion abnormality.The other 13 cases were less than 65% of coronary artery stenosis and stenosis,and no significant segmental wall motion abnormalities were found. The diagnostic accuracy of segmental wall motion abnormality was 97. 30%,98% and 82. 86% respectively,and the specificity and positive predictive value were 100%. The consistency between the diagnostic results of echocardiography and the results of coronary angiography was good（Kappa = 0. 782）. Conclusion： Echocardiography of segmental wall motion abnormalities in patients with coronary heart disease accurately check the diagnostic value of good.
