影响老年隐性高血压发病的危险因素研究

摘　　要：目的探讨影响老年隐性高血压发病的危险因素。方法选取2014年1月至2017年1月期间接诊的420例老年高血压病人,将其中78例隐性高血压病人设为A组。同时选取60例健康人作为对照组。回顾性分析观察对象的各项临床资料,对比2组的临床资料,并通过多元Logistic回归分析探讨影响老年隐性高血压发病的危险因素。结果 A组与对照组体质量指数(BMI)、高血压家族史、总胆固醇(TC)、甘油三酯(TG)、尿酸(UA)、肌酐(Cr)以及丙氨酸氨基转移酶(ALT)与对照组比较,差异均有统计学意义(均P<0.05)。多元Logistic回归分析结果显示,BMI、高血压家族史是老年隐性高血压发病的危险因素(均P<0.05)。结论 BMI和高血压家族史是老年隐性高血压发病的危险因素,临床应针对上述危险因素,重点监控BMI偏高以及有高血压家族史人群的血压水平,及早诊治,以提高本病的防控效果。Objective To explore the risk factors of masked hypertension in the elderly.Methods Four hundred and twenty elderly patients with hypertension from January 2014 to January2017 were selected. Seventy-eight patients with masked hypertension were enrolled in group A. Sixty healthy people were selected as the control group. The clinical data of the subjects were retrospectively analyzed and compared,and the risk factors of masked hypertension in the elderly were analyzed by multiple Logistic regression analysis. Results Compared with control group,the family history of hypertension,body mass index（ BMI）,total cholesterol（ TC）,triglyceride（ TG）,uric acid（ UA）,creatinine（ Cr） and alanine transaminase（ ALT） showed statistically significant differences in group A（ P〈0. 05）. Multivariate Logistic regression analysis showed that BMI,family history of hypertension were risk factors of the incidence of masked hypertension in the elderly（ P〈0. 05）. Conclusions BMI and family history of hypertension are risk factors of the incidence of masked hypertension in the elderly. We should monitor the blood pressure of the patients with high BMI and family history of hypertension,which is helpful to early diagnosis and treatment for masked hypertension and can improve the prevention effect of the disease.
