摘　　要：目的观察外周静脉系统溶栓和导管接触溶栓治疗急性下肢深静脉血栓的疗效,对比哪种治疗方式能有效降低下肢深静脉血栓形成后综合征的发生率。方法将60例急性下肢深静脉血栓患者随机分为导管接触溶栓组和外周静脉系统溶栓组,分别给予经导管接触溶栓和外周静脉系统溶栓治疗,计算CDT组和ST组治疗前、后大腿周胫差及小腿周胫差,6个月后统计两组患者下肢深静脉血栓形成后综合征的发生例数,计算两组患者PTS的发生率。结果 CDT组治疗前、后大腿周径差大于ST组(t=2.215,P <0.05),小腿周径差也大于外周静脉溶栓组(t=2.012,P <0.05);导管接触溶栓组下肢深静脉血栓形成后综合征的发生率低于外周静脉系统溶栓组。结论导管接触溶栓能有效降低下肢深静脉血栓形成后综合征的发生率。Objective To observe the efficacy of thrombolytic treatment of acute iliac venous thrombosis with peripheral venous system and catheter contact, which treatment can effectively reduce the incidence of deep venous thrombosis syndrome in lower extremities. Methods Sixty patients with acute iliac venous thrombosis were randomly divided into catheter contact thrombolytic group and peripheral venous thrombolytic group, and transcatheter contact thrombolysis and peripheral venous thrombolytic treatment were performed, and the anterior and posterior thigh shins of CDT and ST groups were calculated. After 6 months, the incidence of PTS was calculated by counting the cases of patients with deep venous thrombosis in lower extremities in two groups. Results The diameter difference between the anterior and posterior thighs of the CDT group was greater than that of the ST group（t = 2.215, P 〈 0.05）, and the circumference difference of the calf was also greater than that of the peripheral venous thrombolysis group（t = 2.012, P 〈 0.05）; The incidence of lower extremity deep venous thrombosis syndrome was lower than that of peripheral venous thrombolytic group. Conclusion Thrombolysis by catheter contact can effectively reduce the incidence of post-deep venous thrombosis syndrome in lower extremities.
