手足口病患儿血浆脑钠肽和降钙素原检测的临床意义

摘　　要：目的:探讨血清脑钠肽(BNP)和血清降钙素原(PCT)在手足口病病情变化中的临床意义。方法:选择手足口病患儿120例,按是否累及重要脏器分为重症组、普通组各60例,同时另选择健康儿童60例作为正常组。测定各组儿童BNP及PCT水平,并进行分析。结果:3组儿童血浆BNP、PCT水平比较差异均有统计学意义(P<0.001),两两比较显示重症组与普通组儿童差异有统计学意义(P<0.05),重症组与正常组儿童差异有统计学意义(P<0.05),而普通组与正常组儿童差异无统计学意义(P>0.05);重症组患儿的血浆BNP与PCT水平呈正相关(r=0.704,P<0.01);而普通组(r=-0.290,P=0.41)及正常组(r=-0.138,P=0.14)BNP与PCT水平无相关性。结论:手足口病患儿的血清BNP水平与病程进展密切相关,其可作为判断重症手足口病患儿病情严重程度的有效指标,值得进一步推广;对重症患儿,血清BNP和PCT两者联合检测可以有助对患儿预后的评估。Objective：To investigate the clinical significance of serum brain natriuretic peptide （BNP） and serum procalcitonin （PCT） in the change of hand - foot -mouth disease （HFMD）. Methods ：120 children with HFMD were chosen and divided into the severe group and the common group based on the fact whether the disease has spread to important organs. At the same time, 60 healthy children were chosen as the normal group. The levels of BNP and PCT were measured and an- alyzed. Results ：There was difference in BNP and PCT levels among the three groups （ P 〈 0. 001 ） ; there was significant difference between the severe group and the common group （ P 〈0.05） ; there was significant difference between the severe group and the normal group （ P 〈0.05） ; There was no difference between the com- mon group and the normal group （ P 〉 0.05）. The plasma BNP was positively con＇elated with PCT levels in the severe group （ r= 0. 704, P 〈 0.01 ） ; There was no correlation between plasma BNP and PCT levels in the common group （ r = -0.29, P -0.41 ）and the normal group （ r = -138, P -0.14）. Conclusion：The levels of serum BNP in children with HFMD are closely related to the progress of the disease course and can be used as an effective index to judge the severity of the disease, worthy of further promotion. In severe children, the combined measurement of serum BNP and PCT can help to evaluate the prognosis of children.
