摘 要:目的探讨维生素受体(VDR)基因ApaⅠ(rs7975232)位点和TaqⅠ(rs731236)位点多态性与慢性阻塞性肺疾病(慢阻肺)发病率之间的关系,以及炎性因子TNF-α、IL-8在不同VDR基因型中的表达情况。方法收集2015年06月-2016年12月,入住上海市浦东新区人民医院呼吸科,慢阻肺患者80例,同期住院的非慢阻肺患者45例,运用聚合酶链反应-限制性片段长度多态性(PCR-RFLP)技术对其血样DNA进行VDR基因ApaⅠ位点及TaqⅠ位点多态性进行检测,分析其基因型、基因频率,并与临床特征比对,统计分析其相关性。结果慢阻肺患者与非慢阻肺患者(对照组)比较,ApaⅠ位点基因型分布具有差异(χ~2=6.266,P=0.039),慢阻肺患者ApaⅠ基因位点上GT基因型较非慢阻肺患者常见;TaqⅠ位点基因型分布在慢阻肺患者与非慢阻肺患者中无显著差异(χ~2=0.425,P=0.369);ApaⅠ位点及TaqⅠ位点基因型频率与TNF-α、IL-8含量无显著相关性(TNF-α:PApaⅠ=0.979,PTaqⅠ=0.857;IL-8:PApaⅠ=0.747,PTaqⅠ=0.764)。结论慢阻肺发病率与VDR基因ApaⅠ(rs7975232)位点的SNP间存在遗传相关性,GT基因型患者可能更容易发生慢阻肺。Objective To investigate the association between Apa I(rs731236),Taq I(rs7975232) of vitamin D receptor( VDR) gene polymorphism and chronic obstructive pulmonary disease( COPD),and the expression of serum TNF-α and IL-8 level in various VDR genotypes.Methods A total of 80 COPD patients and 45 concurrent patients without COPD were involved in study.All the patients were enrolled from June,2015 to December,2016 in the respiratory department of People's Hospital.By PCR-RFLP technique,the VDR gene polymorphisms of blood samples were tested.Results There was significant difference in genotype frequency between COPD patients and non-COPD patients in VDR gene SNP locus Apa Ⅰ( χ~2= 6.266,P = 0.039),but no significant difference was found in SNP locus Taq Ⅰ( χ~2= 0.425,P = 0.369).The GT genotype of SNP locus Apa Ⅰ was more frequent in COPD patients.The VDR genotype frequency was not relevant with serum TNF-α and IL-8 level( TNF-α:PApaⅠ =0.979,PTaqⅠ = 0.857; IL-8: PApa Ⅰ = 0.747,PTaqⅠ = 0.764).Conclusion COPD morbidity is correlated with VDR gene polymorphism in SNP locus ApaⅠ,and patients with GT genotype are susceptible to COPD.
