津力达颗粒联合通心络胶囊治疗早期糖尿病肾病患者对微炎性状态及肾功能保护的影响

摘　　要：目的 观察津力达颗粒联合通心络胶囊对早期糖尿病肾病患者微炎性状态及肾功能保护作用。方法 选择云南省第二人民医院内分泌科2017年2月—2018年2月间收治的早期2型糖尿病肾病患者128例,按照随机数字表法将患者分为试验组(n=64)与对照组(n=64),每位患者除接受基础糖尿病治疗外,对照组给予厄贝沙坦(安博维),试验组给予津力达颗粒联用通心络胶囊,治疗3个月后,观察比较2组患者微炎性状态、血糖、肾功能的变化,并进行疗效评定。结果 试验组总有效率93.75%高于对照组的70.31%,差异具有统计学意义(χ~2=11.926,P<0.01)。治疗前,2组血清IL-8、TNF-α、hs-CRP、FPG、2hPG、HbA_(1c)、SCr、BUN、24 h尿蛋白总量比较差异无统计学意义(P>0.05)。治疗后,2组患者的各项指标较治疗前均明显降低,差异具有统计学意义(P<0.05),且试验组显著低于同期对照组,差异具有统计学意义(t=13.283、4.865、11.468、6.887、7.174、2.925、8.144、5.370、6.656,P均<0.01)。结论 津力达颗粒联合通心络胶囊治疗早期糖尿病肾病患者,可以显著改善微炎性状态,更好地控制血糖水平与保护肾功能,治疗效果较好,值得推广使用。Objective To investigate micro inflammatory state and renal protection of Jinlida granule combined with Tongxinluo capsule in patients with early diabetic nephropathy. Methods One hundred and twenty eitht patients with early type 2 diabetic nephropathy treated in outpatient from February 2017 to February 2018 were selected,who were randomly divided into experimental group （ n =64） and control group （ n =64）. Each patient received diabetes foundation treatment,the control group was given irbesartan and experimental group were treated with Jinlida granule combined with Tongxinluo capsule. Micro inflammatory state, blood glucose level and renal protection effect of Jinlida Granule combined with Tongxinluo Capsule in patients with diabetic nephropathy were compared. Results The total effective rate of the experimental group was 93.75%,and the effect was better than that of the control group （70.31%）. The difference was statistically significant （χ 2=11.926, P 〈0.01）. There was no significant difference in serum IL-8, TNF-α,hs-CRP, FPG, 2hPG, HbA 1c , SCr, BUN, 24 h urine protein between the 2 groups before treatment （ P 〉0.05）. After different treatment, the indexes of the 2 groups in the experimental group were significantly lower than those before the treatment. The difference was statistically significant （ P 〈 0.05）, and the experimental group was significantly lower than that of the control group （ t =13.283, t =4.865, t = 11.468 , t =6.887, t =7.174, t =2.925, t =8.144, t =5.370, t =9.604, P =0.000, P =0.000, P =0.000, P =0.000, P =0.000, P =0.012, P =0.000, P =0.011, P =0.000）. Conclusion Jinlida granules with Tongxinluo capsule in the treatment of early diabetic nephropathy patients can significantly improve the micro inflammatory state and better control the level of blood glucose and improve renal function. The treatment effect is worthy for further promotion.
