某院万古霉素临床使用合理性分析

摘　　要：目的调查医院住院患者万古霉素的使用情况,分析其用药合理性,为万古霉素临床合理应用和监管提供参考。方法回顾性研究医院2017年1~6月363例使用万古霉素的住院患者病历,分析其药品使用合理性、病原学检查及不良反应发生情况等。结果 363例病历的万古霉素药物利用指数(DUI)=0.92,联合用药146例(40.22%),病原微生物送检率为83.48%,不良反应发生率为1.93%(7例),监测血药质量浓度27例(7.44%)。用药不适宜病历13份(3.58%)。结论该院万古霉素的临床使用基本合理,不存在过度使用的情况,但仍存在个别不适宜情况,今后应加强万古霉素合理使用的监管,提高用药的合理性。Objective To investigate the use of vancomycin in a hospital of inpatient,analyze its rationality and provide basis for the clinical application and supervision of vancomycin. Methods Retrospective survey was conducted for the medical records of 363 inpatients using vancomycin from January to June of 2017 in the hospital and the rationality,etiology and adverse reactions of vancomycin were analyzed. Results The vancomycin drug utilization index （DUI） of the 363 cases was 0.92.146 cases （40.22%） had the drug combination.The microbial test sample submission rate was 83.48%.The rate of adverse reactions was 1.93% （7 cases）. 27 cases （7.44%） patients were conducted therapeutic drug monitoring.There were 13 cases （3.58%） of unsuitable drug use in total. Conclusion The clinical use of vancomycin in this hospital is basically reasonable,there is no excessive use of the situation,but there are still individual inappropriate circumstances.We should strengthen the rational use of vancomycin regulation,and improve the rationality of clinical medication.
