增液润肠汤联合西药治疗糖尿病便秘(阴亏热结)随机平行对照研究

摘　　要：[目的]观察增液润肠汤联合西药治疗糖尿病便秘(阴亏热结)疗效。[方法]使用随机平行对照方法,将118例门诊患者按就诊顺序号简单随机分为两组,饮食和运动计划,控制血糖;多喝水,多食用富含维生素和纤维素蔬菜(丝瓜、芹菜);一般全身运动,重点加强腹肌力量锻炼,多做下蹲与屈髋压腹动作。对照组50例甲钴胺,1片/次,3次/d;多潘立酮,1片/次,3次/d;乳果糖,1瓶/次,1次/d;开塞露,1支/次,便秘严重时使用。治疗组50例胰肾康丸液润肠汤(玄参40g,麦冬、郁李仁、何首乌、地黄~生各15g,当归、芍药各9g,火麻仁15g,甘草3g,木香6g),水煎400mL,1剂/d,早晚分服;西药治疗同对照组。连续治疗1周为1疗程。观测临床症状、便秘消失时间、不良反应。治疗1疗程(1周),判定疗效。随访4周,观测复发率。[结果]治疗组临床痊愈26例,有效32例,无效1例,总有效率98.30%;对照组临床痊愈14例,有效37例,无效8例,总有效率86.44%;治疗组疗效优于对照组(P<0.05)。便秘消失时间治疗组短于对照组(P<0.01)。随访4周,治疗组复发率15.38%低于对照组28.57%(P<0.05)。[结论]增液润肠汤联合西药治疗糖尿病便秘(阴亏热结),疗效满意,无严重不良反应,值得推广。[Objective] To observe the therapeutic effect of Zengye Runchang decoction（增液润肠汤） combined with western medicine on diabetic constipation （Yinkui Rejie）. [Methods] Random parallel control method was used to divide 118 outpatients into two groups according to the sequence number, diet and exercise plan, control blood sugar, drink more water, eat more vitamin and cellulose vegetables （silk gourd and celery）; general exercise, emphasis on strength of abdominal muscle strength, and more squat and hip flexion Abdominal movement. In the control group, 50 cases of mecobalamin, 1 tablets/times, 3 times/d; domperidone, 1 tablets/times, 3 times/d; lactulose, 1 bottles/times, 1 times/d; enema glycerini, 1 Zhi/times, and severe constipation. In the treatment group, 50 cases of Zengye Runchang decoction （Xuanshen 40g, Maidong, Yuliren, Heshouwu, Shengdihuang each 15g, Danggui, Shaoyao each 9g, Huomaren 15g, Gancao 3g, Muxiang 6g）, 400mL, 1 doses/d, morning and evening; western medicine was treated with the control group. Continuous treatment for 1 weeks was the 1 course of treatment. Clinical symptoms, constipation disappearance time and adverse reactions were observed. The 1 course of treatment （1 weeks） was treated and the curative effect was determined. The recurrence rate was observed for 4 weeks. [Results] in the treatment group, 26 cases were cured, 32 cases were effective, 1 cases were invalid, the total effective rate was 98.30%, 14 cases were cured, 37 cases were effective, 8 cases were invalid, the total effective rate was 86.44%, the curative effect of the treatment group was better than the control group （t）〈0.05）. The disappearance time of constipation was shorter in the treatment group than in the control group （P〈0.01）. After 4 weeks of follow-up, the recurrencerate in the treatment group was 15.38% lower than that in the control group 28.57% （P〈0.05）. [Conclusion] Zengye Runehang decoction combined with western medicine in the treatment of diabetic constipation
