肾肝宁胶囊联合来氟米特治疗狼疮性肾炎的临床研究

摘　　要：目的探讨肾肝宁胶囊联合来氟米特片治疗狼疮性肾炎的临床疗效。方法选取2017年2月—2017年9月在资阳市第一人民医院治疗的狼疮性肾炎患者72例,根据用药差别分为对照组(36例)和治疗组(36例)。对照组口服来氟米特片,20 mg/次,1次/d;治疗组在对照组基础上口服肾肝宁胶囊,5粒/次,3次/d。两组均治疗6个月。观察两组患者临床疗效,同时比较治疗前后两组患者24 h尿蛋白定量(24 h UPR)、血肌酐(Scr)、尿素氮(BUN)、尿红细胞(RBC)、金属蛋白酶抑制因子-1(TMP-1)、白细胞介素-33(IL-33)和白细胞介素-34(IL-34)水平。结果治疗后,对照组和治疗组临床有效率分别为77.78%和94.44%,两组比较差异具有统计学意义(P<0.05)。治疗后,两组24 h UPR、Scr、BUN、尿RBC、TMP-1、IL-33和IL-34水平均显著降低(P<0.05),而ALB和MMP-9均显著升高(P<0.05),同组比较差异具有统计学意义(P<0.05);且治疗组这些指标治疗后水平明显优于对照组,两组比较差异具有统计学意义(P<0.05)。结论肾肝宁胶囊联合来氟米特片治疗狼疮性肾炎可有效改善患者临床症状和肾功能,具有一定的临床推广应用价值。Objective To investigate the clinical efficacy of Shenganning Capsules combined with leflunomide in treatment of lupus nephritis. Methods Patients（72 cases） with lupus nephritis in the First People＇s Hospital of Ziyang from February 2017 to September 2017 were divided into control（36 cases） and treatment（36 cases） groups based on different treatments. Patients in the control group were po administered with Leflunomide Tablets, 20 mg/time, once daily. Patients in the treatment group were po administered with Shenganning Capsules on the basis of the control group, 5 grains/time, three times daily. Patients in two groups were treated for 6 months. After treatment, the clinical efficacy was evaluated, and the 24 h UPR, Scr, BUN, RBC, TMP-1, IL-33, IL-34, ALB and MMP-9 levels in two groups before and after treatment were compared. Results After treatment, the clinical efficacy in the control and treatment groups was 77.78% and 94.44%, respectively, and there were differences between two groups（P〈 0.05）. After treatment, the 24 h UPR, Scr, BUN, RBC, TMP-1, IL-33, and IL-34 levels in two groups were significantly decreased, but ALB and MMP-9 levels were significantly increased, and the difference was statistically significant in the same group（P〈 0.05）. And these indexes in the treatment group after treatment were significantly better than those in the control group, with significant difference between two groups（P〈 0.05）. Conclusion Shenganning Capsules combined with leflunomide in treatment of lupus nephritis can effectively improve the clinical symptoms and renal function, which has a certain clinical application value.
