中药熏洗联合曲安奈德封闭及点状电灼治疗慢性肛周湿疹疗效观察

摘　　要：目的对比观察中药熏洗联合曲安奈德封闭及点状电灼治疗慢性肛周湿疹的临床疗效。方法将2016年6月—2017年5月湖北省中医院肛肠科收治的98例慢性肛周湿疹患者按照随机数表法随机分为试验组(48例)与对照组(50例),其中试验组患者于曲安奈德封闭及点状电灼后采用中药熏洗治疗,对照组患者于肛周末梢神经离断术后采用曲安奈德封闭治疗,对比观察两组患者的并发症发生情况、临床疗效及复发情况。结果治疗过程中两组患者并发症发生情况对比采用四格表资料的卡方检验,χ~2=3.204,P=0.031,P<0.05,差异具有统计学意义;治疗4周后两组患者临床疗效对比采用等级资料的秩和检验,mann-whitney U=985.500,Z=-1.968,P=0.049,P<0.05,差异具有统计学意义;随访6个月后两组患者肛周湿疹复发情况对比采用四格表资料的卡方检验,χ~2=3.214,P=0.036,P<0.05,差异具有统计学意义。结论中药熏洗联合曲安奈德封闭及点状电灼治疗慢性肛周湿疹,可有效减少皮下血肿等并发症的发生,促进皮损创面痊愈,降低复发率,疗效确切,具有较高的临床应用价值,值得推广应用。Objective To observe the clinical efficacy of Chinese herbal fumigation and washing in combination with triamcinolone acetonide sealing and punctiform fulguration in the treatment of chronic perianal eczema. Methods Ninety-eight patients with chronic perianal eczema,admitted to Hubei Provincial Hospital of Chinese Medicine from June2016 to May 2017,were randomly divided into an experimental group（48 cases） and a control group（50 cases） according to the random number table. The patients in the experimental group were treated with triamcinolone acetonide sealing and punctiform fulguration,followed by Chinese herbal fumigation and washing,while the patients in the control group were treated with perianal peripheral denervation,followed by triamcinolone acetonide sealing. Complications,clinical efficacy and recurrence were compared and observed in the two groups. Results During the course of treatment,chi-square test of fourfold table was used for the comparison of complications between the two groups and the result showed statistically significant difference（ χ~2= 3. 204,P = 0. 031,P〈0. 05）. After 4 weeks of treatment,rank sum test of ranked data was used for the comparison of the clinical efficacy between the two groups and statistically significant difference was observed（ mann-whitney U = 985. 500,Z =-1. 968,P = 0. 049,P〈0. 05）. During the follow-up of six months,the recurrence of perianal eczema was compared between the two groups with chi-square test of fourfold table and statistically significant difference was observed（ χ~2= 3. 214,P = 0. 036,P〈0. 05）. Conclusion Chinese herbal fumigation and washing in combination with triamcinolone acetonide sealing and punctiform fulguration in the treatment of chronic perianal eczema can effectively avoid the occurrence of complications including subcutaneous hematoma,promote wound healing and decrease the recurrence rate.The combined therapy deserves to be promoted in clinical practice due to its high clinical application value.
