超声导入疤痕软膏联合弹力加压疗法治疗增生性瘢痕的临床疗效研究

摘　　要：目的研究探讨超声导入疤痕软膏联合弹力加压疗法治疗增生性瘢痕的临床疗效。方法将2015年10月—2017年9月中南大学湘雅三医院收治的106例增生性瘢痕患者按照单双号数字表法随机分为研究组(53例)与对照组(53例),其中研究组患者采用超声导入疤痕软膏联合弹力加压疗法治疗,对照组患者采用疤痕软膏外涂按摩联合弹力加压疗法治疗,对比观察两组患者治疗3个月及6个月后的临床疗效、瘢痕疼痛与瘙痒评分以及瘢痕组织中Ⅰ型胶原蛋白的表达水平。结果治疗3个月后,两组患者临床疗效对比采用等级资料的秩和检验,mann-whitney U=1.016E3,Z=-2.666,P=0.008,P<0.01,差异具有统计学意义;治疗6个月后,两组患者临床疗效对比采用等级资料的秩和检验,mann-whitney U=1.074E3,Z=-2.266,P=0.023,P<0.05,差异具有统计学意义;治疗3个月后,两组患者瘢痕疼痛及瘙痒评分对比采用u检验,u=3.410及3.144,P均<0.01,差异具有统计学意义;治疗6个月后,两组患者瘢痕疼痛及瘙痒评分对比采用u检验,u=2.720及2.673,P均<0.01,差异具有统计学意义;治疗3个月及6个月后,两组患者瘢痕组织中Ⅰ型胶原蛋白面密度对比采用u检验,u=13.724及10.703,P均<0.01,差异具有统计学意义。结论超声导入疤痕软膏联合弹力加压疗法治疗增生性瘢痕,可有效缓解瘢痕疼痛及瘙痒,降低瘢痕组织中Ⅰ型胶原蛋白的表达水平,促进瘢痕消退,疗效显著,具有较高的临床应用价值。Objective To investigate the clinical efficacy of ultrasonic introduction of scar ointment combined with elastic compression therapy in the treatment of hypertrophic scar. Methods One hundred and six patients with hypertrophic scar admitted to The Third Xiangya Hospital of Central South University between October 2015 and September 2017 were evenly divided into a study group（ 53 cases） and a control group（ 53 cases） according to the odd-even number table.Patients in the study group were treated with the ultrasonic introduction of scar ointment in combination with the elastic compression therapy while the control group was given the external massage with scar ointment in combination with the elastic compression therapy. After 3 and 6 months of treatment,the clinical efficacy,scar pain and itching scores,and expression level of type Ⅰ collagen in scar tissues were observed and compared between the two groups. Results At 3 and 6 months after treatment,the rank sum test of ranked data was used for the comparison of clinical efficacy between the two groups,which both showed statistically significant differences,mann-whitney U = 1. 016 E3 and 1. 074 E3 respectively,Z =-2. 666 and-2. 266 respectively,P = 0. 008 and 0. 023 respectively,P〈0. 01 and 0. 05 respectively; u test was adopted for the comparison of scar pain and itching scores between the two groups,and the results showed statistically significant differences,u = 3. 410 and 3. 144,both P〈0. 01 at 3 months after treatment while u = 2. 720 and 2. 673,both P〈0. 01 at 6 months after treatment. The surface density of type Ⅰ collagen in scar tissues at 3 and 6 months after treatment between the two groups was compared with u test and the results showed statistically significant differences,u = 13. 724 and 10. 703 respectively,both P〈0. 01. Conclusion The combined therapy of ultrasonic introduction of scar ointment and elastic compression in treating hypertrophic scar can effectively relieve scar pain and itching,reduce the expression level of type
