摘 要:目的:探讨血清白细胞介素(IL)-8、高敏C反应蛋白(hsCRP)、脂蛋白a[Lp(a)]水平检测在冠心病(CHD)患者中的应用价值。方法:选择2013年7月~2016年6月我院收治的CHD患者118例,依据病情分为稳定型心绞痛(SAP)组(29例)、不稳定型心绞痛(UAP)组(47例)和急性心肌梗死(AMI)组(42例)。选择同期在我院参与健康体检的健康人50例纳入健康对照组。检测并分析血清IL-8、hsCRP、Lp(a)与心肌肌钙蛋白I(cTnI)、N末端脑钠肽前体(NT-proBNP)水平的相关性。结果:与健康对照组比较,SAP组、UAP组和AMI组的TC、TG、LDL-C、血清IL-8[(23.22±2.46)ng/L比(30.26±3.22)ng/L比(35.83±3.61)ng/L比(39.27±4.03)ng/L]、hsCRP[(3.38±0.39)mg/L比(4.11±0.52)mg/L比(6.48±0.69)mg/L比(10.53±1.15)mg/L]、Lp(a)[(96.69±9.64)mg/L比(175.76±17.61)mg/L比(241.57±24.21)mg/L比(315.86±32.28)mg/L]、cTnI[(0.36±0.07)μg/L比(3.68±0.18)μg/L比(7.84±0.83)μg/L比(11.97±1.22)μg/L]和NT-proBNP水平[(147.83±15.92)ng/L比(1352.35±142.57)ng/L比(2176.65±212.85)ng/L比(3298.73±330.24)ng/L]均显著升高,且CHD的三个亚组间均符合:SAP组<UAP组<AMI组,两两比较均有显著差异(P<0.05或<0.01)。Spearman相关分析显示,血清IL-8、hsCRP、Lp(a)与cTnI、NTproBNP水平呈显著正相关(r=0.964~0.998,P均=0.001)。结论:IL-8、hsCRP、Lp(a)与CHD的发生发展关系密切,检测血清IL-8、hsCRP、Lp(a)水平对于评价CHD的严重程度具有重要价值。Objective: To explore correlation among serum interleukin (IL) - 8, hsCRP, Lp (a) and occurrence, se- verity of coronary heart disease (CHD). Methods: A total of 118 CHD patients, who were treated in our hospital from Jul 2013 to Jun 2016, were selected. According to their disease condition, they were divided into stable angina pectoris (SAP) group (n = 29), unstable angina pectoris (UAP) group (n = 47) and acute myocardial infarction (AMI) group (n = 42). Another 50 healthy subjects undergoing physical examination during the same period were selected as healthy control group. Levels of cardiac troponin I (cTnI) and N terminal pro brain natriuretic peptide (NT-proBNP) etc. were measured, and their correlation with IL-8, hsCRP, Lp (a) were analyzed. Results: Compared with healthy control group, there were significant rise in levels of TC, TG, LDL-C, serum IL-8 [ (23.22 ±2.46) ng/L vs. (30.26±3.22) ng/L vs. (35.83 ±3.61) ng/L vs. (39.27 ±4.03) ng/L], hsCRP [ (3.38 ±0.39) mg/Lvs. (4.11±0.52) mg/Lvs. (6.48±0.69) mg/Lvs. (10.53±1.15) mg/L], Lp (a) [(96.69±9.64)mg/L vs. (175.76 ±17.61) mg/L vs. (241.57 ±24. 21) mg/L vs. (315.86 ±32.28) mg/L], cTnI [ (0.36 ±0.07) μg/L vs. (3.68 ±0.18) μg/L vs. (7.84 ±0.83) μg/L vs. (11.97 ±1.22) μg/L] and NT-proBNP [- (147.83 ±15.92) ng/L vs. (1352.35 ±142.57) ng/L vs. (2176.65 ±212.85) ng/L vs. (3298. 73 ±330.24) ng/L], and all a- bove indexes accorded with SAP group 〈UAP group〈AMI group, and there was significant difference between any two groups, P〈0.05 or 〈0.01. Spearman correlation analysis indicated that serum IL-8, hsCRP and Lp (a) were significant positively correlated with cTnI and NT-proBNP levels (r = 0. 964~0. 998, P = 0. 001 all). Conclusion: Levels of IL-8, hsCRP and Lp (a) are closely associated with occurrence, severity of CHD .
