中医药治疗表皮生长因子受体抑制剂所致皮疹用药分析

摘　　要：目的:总结中医药治疗表皮生长因子受体抑制剂(epidermal growth factor receptor inhibitors,EGFRIs)所致皮疹的用药规律,评价中医药干预的临床疗效。方法:通过系统检索Pub Med、中国期刊全文数据库、万方数据知识服务平台、中国生物医学文献数据库、维普网等数据库,纳入2007年1月至2017年9月有关中医药治疗表皮生长因子受体抑制剂所致皮疹的临床研究文献,对中药功效类别、用药频次、评价标准和治疗有效率进行分析总结。结果:共有20篇文献符合纳入标准,其中药味完整的方剂共15首,清热药的使用频率最高,为44.8%,其中又以清热解毒药的使用频率较高,其次为补虚药和解表药,使用频率分别为19.4%和14.2%。甘草、金银花、苦参、荆芥和野菊花为使用频次最高的5味中药。在疗效评价中,以美国国家癌症研究所制定的《通用不良事件术语标准》为皮疹评价分级标准。中医药和中西医结合治疗EGFRIs所致皮肤皮疹的疗效显著,且有效率高于常规西医治疗。结论:中医药在治疗表皮生长因子受体抑制剂所致皮疹方面具有明显优势,其用药具有一定规律性,能显著改善临床症状,加速皮疹消退,提高患者生活质量。Objective： To summarize the drug-use rules of Chinese medicine in treating rash induced by epidermal growth factor receptor inhibitors（ EGFRIs）,and to evaluate the clinical efficacy of Chinese medicine intervention. Methods： Through systematic retrieval of Pub Med,Chinese journal full text database,Wanfang Data knowledge service platform,Chinese biomedical literature database,VP network and other databases,the literature on the treatment of rash induced by EGFRIs from January 2007 to September 2017 were included. Efficacy category,frequency of use,evaluation criteria and effective rate of treatment of traditional Chinese Medicine were analyzed and summarized. Results： A total of 20 articles were in accordance with the inclusion criteria,of which 15 of the prescriptions were complete,the antipyretic drugs had the highest use frequency： 44. 8%,and the use frequency of the antipyretic drugs was higher,followed by drugs for the supplement of deficiency and exterior syndrome relieving Chinese medicinal herbs,the frequency of use was 19. 4% and 14. 2% respectively. Gancao（ Radix et Rhizoma Glycyrrhizae）,Jinyinhua（ Flos Lonicerae Japonicae）,Kushen（ Radix Sophorae Flavescentis）,Jingjie（ Herba Schizonepetae）,Yejuhua（ Flos Chrysanthemi Indici） are the 5 most frequently-used Chinese herbs. In terms of the evaluation of curative effect,the standard of general adverse event terminology formulated by the National Cancer Institute of the United States was regarded as the grading standard of rash. The curative effect of TCM,Integrated traditional Chinese medicine and Western medicine on skin rash induced by EGFRIs is remarkable,and the effective rate is higher than that of conventional Western medicine. Conclusion： Traditional Chinese medicine has obvious advantages in the treatment of skin rash caused by epidermal growth factor receptor inhibitor,and its drug use has certain regularity.
