尿沉渣细菌定量计数筛查对于泌尿系感染诊断的临床价值

摘　　要：目的探讨尿沉渣细菌定量计数筛查对于泌尿系感染诊断的临床价值。方法选取本院收治的400例疑似泌尿系统感染患者,按筛查诊断方法分为研究组与对照组各200例。研究组采用尿沉渣细菌定量计数,对照组采用尿细菌培养。将尿细菌培养的检测结果作为"金标准",分析尿沉渣细菌定量计数的灵敏度、特异度、阳性率和阴性率,并评估灵敏度和特异度的可靠性。结果研究组和对照组的阳性率分别为57%和61%,差异无统计学意义(P>0.05);研究组相对于对照组的灵敏度为88.52%,特异度为92.31%,阳性率为94.74%,阴性率为83.72%,Kappa值为0.830,一致性强。结论用尿沉渣细菌定量计数代替中段尿细菌培养,结果准确、可靠。Objective To discuss the clinical value of urine sedimentation bacteria quantitative count for diagnosis of urinary tract infection. Methods A total of 400 patients with suspected urinary tract infection were selected and divided into study group and control group according to the screening diagnosis method,with 200 cases in each group. The study group was given urine sedimentation bacteria quantitative count,and the control group was given urinary bacteria culture. The positive rates were compared between the two methods. The sensitivity,specificity,positive rate and negative rate of urinary sediment count relative to urinary bacterial culture were analyzed,and the reliability of the sensitivity and specificity were evaluated with the results of urine bacteria culture as ＂ gold standard＂. Results The positive rates of study group and control group were 57% and 61%,and the differences were not statistically significant（ P〉0. 05）. The sensitivity of urine sedimentation was 88. 52%,the specificity was 92. 31%,the positive rate was 94. 74% and the negative rate was 83. 72%. The consistency test showed that the Kappa value was 0. 830 and the consistency was the strongest. Conclusion The results of urine sedimentation bacteria quantitative count are accurate and reliable instead of the middle urinary bacteria culture.
