多巴胺联合硝普钠持续静脉泵入治疗老年顽固性心衰的治疗效果及对心功能的影响评价

摘　　要：目的研究多巴胺联合硝普钠持续静脉泵入治疗老年顽固性心衰的临床效果,并分析其对心功能影响作用。方法选取本院2016年9月~2017年8月收治的90例老年顽固性心衰患者,依据入院顺序单双号分为参照组和实验组,每组45例。参照组予以常规治疗法,实验组在常规疗法基础上予以多巴胺联合硝普钠持续静脉泵入,比较两组治疗效果以及治疗前后心功能指标改善程度。结果实验组的治疗总有效率93.33%高于参照组的71.11%(P<0.05)实验组每搏指数、每搏输出量、左室收缩末期内径以及左室射血分数等心功能指标的改善程度均优于参照组,差异有统计学意义(P<0.05)。两组患者在治疗期间均未见严重不良反应。结论应用多巴胺联合硝普钠持续静脉泵入治疗老年顽固性心衰可获得显著疗效,对改善患者心功能具有积极作用,值得推广。Objective To study the clinical effect of continuous intravenous infusion of dopamine combined with sodium nitroprusside in the treatment of refractory heart failure in the elderly, and to analyze its effect on cardiac function. Methods Atotal of 90 elderly patients with refractory heart failure admitted to our hospital from September 2016 to August 2017 were selected and divided into the reference group and the experimental group according to the order of admission, with 45 cases in each group. The control group was treated with conventional therapy, and the experimental group was treated with continuous intravenous infusion of dopamine combined with sodium nitroprusside on the basis of conventional thera- py. Results The total therapeutic efficiency of the experimental group was 93.33% higher than that of the reference group （71.11%）, and the im- provement degree of the cardiac function indicators such as stroke index, stroke output, left ventricular end-point diameter and left ventricular ejection fraction in the experimental group was better than that of the reference group （P 〈 0.05）. No serious adverse reactions were observed during the treatment. Conclusion The application of dopaanine combined with continuous intravenous infusion of nitroprusside sodium in the treatment of refractory heart failure in the elderly can obtain significant curative effect, which has positive effects on improving cardiac function in patients and is worthy of promotion.
