糖化血红蛋白HbA1c检测在糖尿病中的临床价值

摘　　要：目的研究糖尿病患者糖化血红蛋白HbA1c检测的临床价值。方法选取我院35例糖尿病患者为糖尿病组,另选同期正常体检者为正常组。测定并比较两组受检者的空腹血糖(FPG)、HbA1c水平,分析HbA1c与FPG的相关性,并分析HbA1c诊断糖尿病的价值。结果糖尿病组患者的FPG、HbA1c水平均显著高于正常组(P<0.05)。糖尿病组HbA1c与FPG的Pearson相关性系数=0.862,两者极强相关(P=0.000)。HbA1c诊断糖尿病的灵敏度为91.43%(32/35),特异度为94.29%(33/35),漏诊率为8.57%(3/35),误诊率为5.71%(2/35)。结论糖化血红蛋白HbA1c诊断糖尿病的灵敏度、特异度高,与血糖水平有良好相关性,可作为糖尿病早期筛查的指标之一。Objective To study the clinical value of detection of glycosylated hemoglobin HbA1c in patients with diabetes mellitus. Methods Thirty-five patients with diabetes mellitus in our hospital were selected as diabetic group, and35 cases of healthy controls at the same period were selected as normal group. The levels of fasting plasma glucose（FPG）and HbA1c in the two groups were measured and compared. The correlation between HbA1c and FPG were analyzed, and the value of HbA1c in diagnosis of diabetes mellitus were analyzed. Results The FPG and HbA1c in the diabetic group were significantly higher than those in the normal group（P〈0.05）. The Pearson correlation coefficient of HbA1c and FPG in the diabetic group was 0.862, which was strongly correlated（P =0.000）. The sensitivity of HbA1c in the diagnosis of diabetes mellitus was 91.43%（32/35）, the specificity was 94.29%（33/35）, the missed diagnosis rate was 8.57%（3/35）, and the misdiagnosis rate was 5.71%（2/35）. Conclusion Glycated hemoglobin HbA1c has high sensitivity and specificity in the diagnosis of diabetes mellitus, and has a good correlation with level of blood glucose. It can be used as one of the indicators for early screening of diabetes.
