腹膜代阴道与乙状结肠代阴道成形术治疗先天性无阴道的近期效果及术后性生活质量比较

摘　　要：目的探讨腹膜代阴道与乙状结肠代阴道成形术治疗先天性无阴道的近期效果及术后性生活质量。方法选取2014年1月—2016年12月收治的82例先天性无阴道患者作为研究对象。根据手术方案将患者分为对照组40例和观察组42例,对照组行腹腔镜下腹膜代阴道成形术治疗,观察组行腹腔镜下乙状结肠代阴道成形术治疗。治疗结束后比较两组的近期临床疗效和术后性生活质量,并记录术后并发症的发生情况。结果观察组的手术时间、术后进食时间和术后住院时间均明显长于对照组,而术中出血量和术后分泌物异味患者较对照组多(P<0.01)。两组术后女性性功能指数量表中各方面评分、总评分以及并发症总发生率比较差异均无统计学意义(P>0.05)。结论腹腔镜下腹膜代阴道成形术治疗先天性无阴道的近期临床效果较好,更具有微创性,且术后性生活质量尚可,具有一定的安全性。Objective To investigate short-term efficacy and quality of postoperative sexual life between peritoneum and sigmoid colon vaginoplasty in treatment of congenital absence of vagina. Methods A total of 82 patients of congenital absence of vagina admitted during January 2014 and December 2016 were divided into control group （ n =40） and observation group （ n =42） according to operative plans. Control group was treated with laparoscopic hypogastric peritoneum vaginoplasty, while observation group was treated with laparoscopic sigmoid colon vaginoplasty. After treatment, short-term clinical efficacy and quality of postoperative sexual life were compared, and incidence rates of postoperative complications was recorded in two groups. Results In observation group, operative time, postoperative eating and hospitalization times were significantly longer, while values of intraoperative bleeding volume and numbers of patients with postoperative secretion of abnormal flavor were larger than those in control group （ P 〈0.01）. There were no significant differences in postoperative scores and the total score of sexual functioning indexes and the total incidence rate of complications in females between two groups （ P 〉0.05）. Conclusion Laparoscopic hypogastric peritoneum vaginoplasty in treatment of congenital absence of vagina may achieve better short-term efficacy, more minimally invasive and acceptable quality of postoperative sexual life with a certain safety.
