电针八髎穴联合腹部按摩治疗原发性痛经临床观察

摘　　要：目的观察电针八髎穴联合腹部按摩对原发性痛经的临床疗效,及对患者疼痛程度、月经期外周血浆中前列腺素E_2(PGE_2)与前列腺素F2α(PGF2α)含量的变化,探讨其作用机制。方法将64例原发性痛经患者随机分为治疗组和对照组各32例。治疗组采用电针八髎穴联合腹部按摩治疗,于月经周期前1周开始治疗,直至月经周期结束。对照组采用口服布洛芬缓释胶囊,于月经来潮前2d或开始有症状时开始服用,连服3~5d或至症状缓解。两组均治疗3个月经周期,治疗结束后观察疗效。结果治疗结束后,治疗组总有效率(90.63%)明显优于对照组(78.13%),有显著性差异(<0.05);两组治疗后,痛经症状评分显著降低(<0.05),且治疗组优于对照组;治疗结束后,治疗组PGF2α含量升高,PGE_2含量降低,与对照组比较有显著性差异(<0.05)。结论电针八髎穴联合腹部按摩治疗原发性痛经,疗效确切,与口服布洛芬胶囊比较,具有疗效确切、易于掌握、安全方便等优点。Objective To observe the clinical effect of electroacupuncture（EA） at Baliao acupoint combined with abdominal massage on primary dysmenorrhea（PD）,and the change of PGE2 and PGF2α content in pain level and menstrual period peripheral plasma to explore its possible mechanism. Methods 64 cases of patients with PD were randomly divided into treatment group and control group,32 cases in each group. Treatment group was treated with EA at Baliao acupoint combined with abdominal massage,the treatment was started 1 week before the menstrual cycle till the end of the menstrual cycle. In control group,oral ibuprofen sustained-release capsules were used,the treatment was started 2 d before the onset of menstruation or when symptoms began,taking 3～5 d continuously till the symptoms resolved. Both groups were treated for 3 menstrual cycles,and the effects were observed after treatment. Results After treatment,the total effective rate in treatment group（90.63%） was significantly higher than that in control group（78.13%）with significant difference（P〈0.05）. The scores of dysmenorrhea was significantly decreased in both groups（P〈0.05）,and treatment group was lower than that of control group. The content of PGF2α in treatment group increased,and the content of PGE2 decreased,there was significant difference compared with control group（P〈0.05）. Conclusion EA at Baliao acupoint combined with abdomen massage is effective in treatment of primary dysmenorrhea,which is superior to oral ibuprofen capsules.
