益气固肾汤治疗肾病综合征

摘　　要：目的:观察自拟益气固肾汤辨证加减治疗肾病综合征患者的临床疗效。方法:500例肾病综合征患者随机分为观察组和对照组,每组均为250例。对照组给予肾上腺皮质激素治疗,观察组在对照组治疗基础上给予自拟益气固肾汤辨证加减治疗。比较两组患者临床疗效、生化指标(包括血胆固醇、血白蛋白和24小时尿蛋白)和药物不良反应发生率(包含恶心呕吐、满月脸、肝功损害、多毛和感染)。结果:观察组有效率(显效+有效)为97.2%,对照组有效率为78.8%,两组比较,差异有统计学意义(P<0.01)。观察组胆固醇为(5.23±0.97)mmol·L^(-1)和24小时尿蛋白为(1.89±1.96)g,均明显低于对照组[(6.65±1.96)mmol·L^(-1)和(2.98±2.23)g],观察组血白蛋白[(27.37±5.02)g·L^(-1)]显著高于对照组[(25.87±4.98)g·L^(-1)],差异均有统计学意义(P<0.01)。观察组的恶心呕吐(5.2%)、满月脸(39.6%)、肝功损害(21.2%)、多毛(31.6%)和感染(23.6%)发生率远远低于对照组的13.2%、55.2%、39.6%、44.8%和34.4%,差异均有统计学意义(P<0.01)。结论:自拟益气固肾汤辨证加减治疗肾病综合征疗效显著,可明显降低胆固醇和24小时尿蛋白水平,升高血白蛋白水平,且不良反应少。Objective： To observe the clinical efficacy of self-made Yiqi Gushen Decoction in treatment of nephrotic syndrome.Methods： 500 patients with nephrotic syndrome were randomly divided into observation group and control group,with 250 cases in each group. The control group was treated with adrenal cortex hormone,and the observation group was given the treatment of selfmade Yiqi Gushen Decoction in addition to that of the control group. The clinical efficacy,biochemical parameters（ including blood cholesterol,serum albumin and 24-hour urine protein） and the incidence of adverse drug reactions（ including nausea and vomiting,full moon face,liver function damage,hairy hair and infection） were compared between the two groups. Results： The effective rate was 97. 2% in the observation group and that was 78. 8% in the control group. The difference was statistically significant（ P〈0. 01）. The cholesterol of the observation group was（ 5. 23 ± 0. 97） mmol·L-1 and 24-hour urine protein was（ 1. 89 ±1. 96） g,which was significantly lower than that,namely,（ 6. 65 ± 1. 96） mmol·L-1 and（ 2. 98 ± 2. 23） g of the control group,respectively. Serum albumin（ 27. 37 ± 5. 02） g·L-1 was significantly higher than that,namely,（ 25. 87 ± 4. 98） g·L-1 of the control group,and the difference was statistically significant（ P〈0. 01）. The incidence of nausea and vomiting（ 5. 2%）,full moon face（ 39. 6%）,liver function damage（ 21. 2%）,hairy（ 31. 6%） and infection（ 23. 6%） in the observation group was much lower than that,namely,13. 2%,55. 2%,39. 6%,44. 8%,and 34. 4%,of the control group,and the differences were statistically significant（ P〈0. 01）. Conclusion： Self-made Yiqi Gushen Decoction has a significant effect on patients with nephrotic syndrome,which can significantly reduce cholesterol and 24-hour urine protein levels,increase serum albumin levels,and have fewer adverse reactions.
