摘  要:目的探讨Th17/Treg细胞及相关细胞因子血清IL-6、IL-10、IL-17、IL-23、转化生长因子-β(TGF-β)与宫颈癌HPV感染类型的关系,旨在进一步揭示宫颈癌的发病机制。方法选取2016年1月-2018年1月该院收治的97例HPV感染的宫颈病变患者为研究对象,根据病变情况分为宫颈癌(CC)患者(CC组,39例)、Ⅱ~Ⅲ期宫颈上皮内瘤变(CIN)患者(CIN组,25例)、慢性宫颈炎(CCS)患者(CCS组,33例),并根据HPV分型将其分为HPV高危型组(61例)和HPV低危型组(36例)。采用流式细胞仪检测Th17/Treg细胞,酶联免疫吸附法测定相关细胞因子。结果 CC组CD4+IL17+Th17百分比、CD4+CD25+Treg百分比、Th17/Treg高于CCS组(均P<0.05)、CIN组高于CCS组(均P<0.05),且CC组高于CIN组(均P<0.05)。CC组血清IL-6、IL-10、IL-17、IL-23、TGF-β水平高于CCS组(均P<0.05),CIN组高于CCS组(均P<0.05),且CC组高于CIN组(均P<0.05)。HPV高危型组CD4+IL17+Th17百分比、CD4+CD25+Treg百分比、Th17/Treg均高于HPV低危型组(均P<0.05)。HPV高危型组患者血清IL-6、IL-10、IL-17、IL-23、TGF-β水平均高于HPV低危型组(均P<0.05)。结论Th17/Treg细胞在高危型HPV感染患者中存在高表达,Th17/Treg细胞比例失衡参与了宫颈癌的发生发展过程,并且与HPV感染的免疫逃逸密切相关。Objective To explore the relationships between Th17/Treg cells,related cytokines( serum IL-6,IL-10,IL-17,IL-23,and TGF-β) and human papillomavirus( HPV) infection types in patients with cervical cancer,further reveal the mechanism of cervical cancer. Methods From January 2016 to January 2018,97 cervical lesions patients with HPV infection were selected as study object,then they were divided into cervical cancer group( CC group,39 patients),Ⅱ-Ⅲ stage cervical intraepithelial neoplasia group( CIN group,25 patients),and chronic cervicitis group( CCS group,33 patients). All the patients were divided into high-risk HPV group( 61 patients) and low-risk HPV group( 36 patients) according to HPV types. Flow cytometry was used to detect Th17/Treg cells,and ELISA was used to detect related cytokines. Results The percentages of CD4+IL17+Th17 cells,CD4+CD25+Treg cells,and Th17/Treg cells in CC group and CIN group were statistically significantly higher than those in CCS group( P〈0. 05). The percentages of CD4+IL17+Th17 cells,CD4+CD25+Treg cells,and Th17/Treg cells in CC group were statistically significantly higher than those in CIN group( P〈0. 05). The levels of serum IL-6,IL-10,IL-17,IL-23,and TGF-β in CC group and CIN group were statistically significantly higher than those in CCS group( P〈0. 05). The levels of serum IL-6,IL-10,IL-17,IL-23,and TGF-β in CC group were statistically significantly higher than those in CIN group( P〈0. 05). The percentages of CD4+IL17+Th17 cells,CD4+CD25+Treg cells,and Th17/Treg cells in high-risk HPV group were statistically significantly higher than those in low-risk HPV group( P〈0. 05). The levels of serum IL-6,IL-10,IL-17,IL-23,and TGF-β in high-risk HPV group were statistically significantly higher than those in low-risk HPV group( P〈0. 05).
