腹腔镜手术联合促性腺激素释放激素激动剂治疗子宫内膜异位症的疗效及对患者生殖内分泌激素水平的影响

摘　　要：目的探讨腹腔镜手术联合促性腺激素释放激素激动剂(Gn RH-α)治疗子宫内膜异位症(EMS)的疗效及对患者生殖内分泌激素水平的影响,为临床治疗提供参考依据。方法选取2013年1月-2015年1月在南京医科大学附属妇产医院实施腹腔镜手术的EMS患者120例为研究对象,采用SPSS 16. 0生成随机数字表分为Gn RH-α组、孕三烯酮组各60例,两组患者腹腔镜术后应用对应药物治疗,对比两组临床疗效及术后激素水平变化。结果治疗前,Gn RH-α组和孕三烯酮组患者的血清卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E2)、糖类抗原125(CA125)、催乳素(PRL)水平、视觉模拟评分(VAS评分)、子宫内膜厚度差异无统计学意义(均P>0. 05);治疗后,Gn RH-α组患者FSH、LH、E2水平、VAS评分、子宫内膜厚度均低于孕三烯酮组,差异有统计学意义(均P<0. 05)。治疗后,Gn RH-α组缓解率90. 00%高于孕三烯酮组76. 77%,差异有统计学意义(P<0. 05),Gn RH-α组复发率5. 00%低于孕三烯酮组16. 67%,差异有统计学意义(P<0. 05);Gn RH-α组不良反应发生率21. 67%高于孕三烯酮组8. 33%,差异有统计学意义(P<0. 05)。结论腹腔镜手术联合Gn RH-α治疗EMS的疗效较好,但是不良反应发生率相对较高。Objective To explore the curative effect of laparoscopic surgery combined with gonadotropin-releasing hormone angonist（ Gn RHa） in treatment of endometriosis and the influence on levels of reproductive endocrine hormones,provide a reference basis for clinical treatment. Methods From January 2013 to January 2015,120 patients with endometriosis treated by laparoscopic surgery in Affiliated Obstetrics and Gynecology Hospital of Nanjing Medical University were selected as study object,SPSS 16. 0 software was used to analyze the data,all the patients were divided into Gn RHa group and gestrinone group according to random nubmer table,60 patients in each group. The patients in the two groups were treated by corresponding drugs after laparoscopic surgery. The clinical effects and changes of postoperative hormones levels in the two groups were compared. Results Before treatment,there was no statistically significant difference in the levels of serum follicle-stimulating hormone（ FSH）,luteinizing hormone（ LH）,estradiol（ E2）,carbohydrate antigen 125（ CA125）,prolactin（ PRL）,VAS scores,and endometrial thickness between Gn RHa group and gestrinone group（ P〉0. 05）. After treatment,the levels of serum FSH,LH,E2,VAS score,and endometrial thickness in Gn RHa group were statistically significantly lower than those in gestrinone group（ P〈0. 05）. After treatment,the remission rate in Gn RHa group was 90. 00%,which was statistically significantly higher than that in gestrinone group（ 76. 77%）（ P〈0. 05）,the recurrence rate in Gn RHa group was 5. 00%,which was statistically significantly lower than that in gestrinone group（ 16. 67%）（ P〈0. 05）. The incidence rate of adverse reactions in Gn RHa group was 21. 67%,which was statistically significantly higher than that in gestrinone group（ 8. 33%）（ P〈0. 05）. Conclusion The curative effect of laparoscopic surgery combined with Gn RHa in treatment of endometriosis is good,but the incidence rate of adverse reactions is relativel
