摘　　要：右美托咪啶(Dexmedetomidine,DEX)是一种高选择性α2肾上腺素能受体(Adrenergic receptor,AR)激动剂,具有镇静兼部分镇痛、抗焦虑的作用,在围手术期应用广泛。近几年关于DEX镇痛作用的研究报道逐渐增多,围手术期辅以DEX可减少麻醉镇痛药用量,延长镇痛时效,并降低不良反应发生率。该文就DEX的药理特性、镇痛作用机制以及DEX在术后静脉镇痛、椎管内镇痛、外周神经阻滞和关节内镇痛等方面的临床应用作一综述,以系统全面了解DEX在围手术期镇痛方面的应用进展。Dexmedetomidine（ DEX） is a highly seleCTive agonist of alpha-2 adrenergic receptor（ AR） with properties of sedation,partial analgesia and anti-anxiety. It is widely used in perioperative period. In recent years,the reports on the DEX＇s analgesic effeCT have gradually increasing. Perioperative period supplemented with DEX can reduce the dosage of narcotic analgesics,prolong the duration of analgesia and decrease the incidence of adverse events. This paper will review the DEX＇s pharmacological properties,the mechanism of analgesic effeCT as well as the clinical application in postoperative intravenous analgesia,spinal analgesia,peripheral nerve block and intra-articular analgesia to systematically and comprehensively summarize the progress of DEX＇s application in perioperative analgesia.
