射频消融联合针刀对腰椎间盘突出症疼痛症状的改善效果及1年随访研究

摘　　要：目的:探讨射频消融联合针刀对腰椎间盘突出症疼痛症状的改善效果以及术后1年的随访研究。方法:选取60例腰椎间盘突出症,随机分为观察组和对照组各30例。对照组采用针刀治疗,观察组采用射频消融联合针刀治疗。对比两组患者术后10天、术后3个月和术后1年的直腿抬高试验、日本骨科协会评估治疗评分(JOA)、疼痛视觉模拟评分、Oswestry功能障碍评分的评估结果。结果:观察组术后10天的直腿抬高角度为(61.05±10.13)°、JOA评分为(16.95±5.06)分,术后3个月的直腿抬高角度为(75.08±9.71)°、JOA评分为(25.11±3.79)分,术后1年的直腿抬高角度为(84.35±4.59)°、JOA评分为(26.02±3.17)分,均显著高于对照组(P<0.05);观察组术后10天的疼痛视觉模拟评分为(1.42±0.83)分、Oswestry功能障碍评分为(35.19±5.97)分,术后3个月的疼痛视觉模拟评分为(2.01±1.01)分、Oswestry功能障碍评分为(26.71±6.17)分,术后1年的疼痛视觉模拟评分为(1.93±1.18)分、Oswestry功能障碍评分为(26.91±6.08)分,均显著低于对照组(P<0.05)。结论:射频消融联合针刀可迅速、有效缓解腰椎间盘突出症患者的疼痛症状,恢复腰椎的生理功能,且中远期疗效稳定。Objective：To investigate the effect of radio-frequency ablation combined with acupotomy on the pain symptoms of lumbar disc herniation and a 1-year follow-up study.Methods：60 cases of lumbar disc herniation were randomly divided into the observation group and the control group,30 cases in each group.The control group was treated with acupotomy,while the observation group was treated with radio-frequency ablation combined with acupotomy.The evaluation of straight leg raising test,Japanese Orthopaedic Association Scores（JOA）,visual analogue scale and Oswestry disability index 10 days,3 months and 1 years after the operation were compared between the two groups.Results：The straight leg raising test of the observation group was（61.05±10.13）° and the JOA score was（16.95±5.06） points on 10 days after operation,the straight leg raising test of the observation group was（75.08±9.71）° and JOA score was（25.11±3.79） points on 3 months after operation,and the straight leg raising test was（84.35±4.59）° and JOA score was（26.02±3.17） points on 1 years after operation,which were significantly higher than those of the control group（P〈0.05）.The visual analogue scale of the observation group was（1.42±0.83）points and the Oswestry disability index was（35.19±5.97） points on 10 days after operation,the visual analogue scale of the observation group was（2.01±1.01） points and the Oswestry disability index was（26.71±6.17） points on 3 months after operation,and the visual analogue scale of the observation group was（1.93±1.18） points and the Oswestry disability index was（26.91±6.08） points on 1 year after operation,which were significantly lower than those of the control group（P〈0.05）.Conclusion：Radio-frequency ablation combined with acupotomy can quickly and effectively relieve the pain symptoms of the patients with lumbar disc herniation,and restore the physiological function of the lumbar spine,and the medium and long term effect is stable.
