益气降糖饮联合胰岛素对妊娠期糖尿病孕妇血糖、内脂素水平及妊娠结局的影响

摘　　要：目的:探讨益气降糖饮联合胰岛素对妊娠期糖尿病孕妇血糖、内脂素水平及妊娠结局的影响。方法:将124例GDM患者作为研究对象,随机分为对照组(62例)与观察组(62例)。对照组患者给予饮食控制、运动指导和胰岛素注射治疗,观察组患者在此基础上联合益气降糖饮治疗,14d为1个疗程。对两组患者血糖、内脂素水平及妊娠结局进行对比分析,并评价临床治疗效果。结果:治疗前,两组FPG、2hFPG、HbA1C、血清内脂素水平和中医证候积分无差异(P>0.05);治疗后,两组FPG、2hFPG、HbA1C、血清内脂素水平和中医证候积分均有明显下降,观察组明显低于对照组(P<0.05)。观察组治疗总有效率90.32%明显高于对照组75.81%(P<0.05)。观察组剖宫产率,羊水异常、子痫前期、妊娠期高血压、酮症酸中毒等发生率均明显低于对照组(P<0.05)。观察组巨大儿、呼吸窘迫、早产、高胆红素血症等发病率明显低于对照组(P<0.05)。结论:益气降糖饮联合胰岛素对妊娠期糖尿病孕妇具有良好的治疗效果,能够显著控制血糖和内脂素水平,有效改善妊娠结局。Objective： To investigate the effect of the Yiqi Jiangtang decoction and insulin therapy on blood glucose,alfatin level and pregnancy outcome in patients with gestational diabetes.Methods： A total of 124 GDM patients selected and randomly divided into control group （62 cases） and observation group （62 cases）.The control group were treated with diet control,exercise guidance and insulin treatment,the observation group based on the combined use of the Yiqi Jiangtang Decoction . 14d for 1 courses.The blood glucose,the level of endomalin and the outcome of pregnancy were compared and analyzed in the two groups,and the effect of clinical treatment was evaluated.Results： Before treatment,there was no difference between the two groups of FPG,2h FPG,HbA1C,serum lipoprotein level and TCM syndrome score （ P 〉0.05）.After treatment,the levels of FPG,2h FPG,HbA1C,serum visfatin and TCM syndrome scores in the two groups were significantly decreased,which was significantly lower in the observation group than in the control group （ P 〈0.05）.The total effective rate of treatment in the observation group was 90.32% significantly higher than that of the control group （75.81%）（ P 〈0.05）.The rate of cesarean section,abnormal amniotic fluid,preeclampsia,pregnancy induced hypertension and ketoacidosis were significantly lower in the observation group than in the control group （ P 〈0.05）. The incidence of macrosomia,respiratory distress,preterm labor and hyperbilirubinemia in the observation group was significantly lower than that of the control group （ P 〈0.05）.Conclusion： The Yiqi Jiangtang decoction combined with insulin therapy has a good therapeutic effect on pregnant women with gestational diabetes mellitus,which can significantly control the level of blood sugar and visfatin,and effectively improve the outcome of pregnancy.
