智能专家系统的构建及其在性功能障碍诊断真实世界的初步临床应用

摘　　要：目的:探讨智能专家系统在性功能障碍(SD)诊断真实世界中的构建和应用价值。方法:构建智能专家系统"男性健康助手",收集2018年2月6日~2018年6月8日在我院男科门诊就诊的SD患者资料,比较智能专家系统和专家诊断的一致性、特异度和灵敏度等。结果:初步构建了智能专家系统"男性健康助手",入组患者559例,20~39岁占82.47%。智能组评估显示,勃起功能障碍(ED)430例,其中轻度217例;早泄(PE)256例,其中原发性PE(LPE)186例;可能存在抑郁298例。专家组评估显示:ED 385例,PE 239例,其中LPE为95例,伴随精神症状患者17例。智能专家系统相对于专家:诊断ED的一致性、灵敏度、特异度分别为89.45%(Kappa系数0.735,P<0.001)、98.2%、70.1%;诊断PE的一致性、灵敏度、特异度分别为82.29%(Kappa系数0.652,P<0.001)、95.0%、72.8%;诊断LPE的一致性、灵敏度、特异度分别为81.57%(Kappa系数0.527,P<0.001)、93.7%、79.1%。智能组较专家组诊断阳性率显著提高。结论:智能专家系统能改善男科疾病诊疗效率,提高SD的诊断率并显示疾病程度、分型等,有较好的应用前景。Objective：To explore the construction and application value of intelligent expert system in the real world of diagnosis of sexual dysfunction（SD）.Method：The intelligent expert system ＂ male health assistant＂ was constructed to collect the data of patients who were admitted to the male clinic of our hospital as SD from February6 th,2018 to June 8 th,2018.The consistency,specificity and sensitivity of the intelligent expert system and the expert diagnosis were compared.Result：The intelligent diagnostic system,male health assistant was initially constructed,and 559 patients were enrolled,accounting for 82.47% of 20-39 years old.The intelligent group assessment showed 430 cases of erectile dysfunction（ED）,of which 217 cases were mild and 256 cases of premature ejaculation（PE）,of which 186 cases of primary PE（LPE）and 298 cases of depression;The expert group assessment showed 385 cases of ED and 239 cases of PE,of which LPE was 95,and 17 patients were accompanied by mental symptoms.The consistency,sensitivity,and specificity of the intelligent expert system compared to the expert group in the diagnosis of ED were 89.45%,Kappa coefficient 0.735（P〈0.001）,98.2%,70.1%;PE were 82.29%,Kappa coefficient 0.652（P 0.001）.95.0%,72.8%;LPE were 81.57%,Kappa coefficient0.527（P〈0.001）,93.7%,79.1%.The positive rate of the intelligent group was significantly higher than that of the expert group.Conclusion：The artificial intelligent expert system can improve the efficiency of diagnosis and treatment of andrology diseases,increase the diagnosis rate of SD and show the degree and classification of diseases,which has a good application prospect.
