摘　　要：目的提高手术切皮前Time-out正确执行率。方法成立品管圈小组,对手术切皮前Time-out执行率现状进行调查,认为医护人员核查不规范、巡回护士监督不到位、医护人员意识不强为导致问题产生的主要症结。经过解析,发现真因为:制度/流程不完善,缺乏安全意识,缺乏系统培训。对此,多方拟定对策,从完善制度/流程,加强安全意识教育,强化培训与考核3方面改进。结果手术切皮前Time-out正确执行率由66%提高至98%。结论开展品管圈活动,可有效提高手术切皮前Time-out正确执行率,减少手术差错事故发生,降低手术风险,确保手术安全。Objective To improve the correct execution rate of Time-out before surgical incision. Methods A quality control circle group was set up to investigate the current situation of the Time-out implementation rate before surgical incision. It was considered that the important causes of the problem included medicial staffverification is not stansard,the roving nurse supervision is not in place,and the not enough awareness of medical staff. After analysis, it was discovered that the true causes included the imperfect system/process, lack of security awareness, and lack of systematic training. In this regard, various parties had formulated countermeasures, and improved from improving the system/process, strengthening safety awareness education, and strengthening training and assessment. Results The correct implementation rate of Time-out before surgical incision increased from 66% to 98% after improvement. Conclusion Conducting quality control circle activities can effectively improve the correct execution rate of Time-out before surgical incision, reduce the occurrence of surgical errors, reduce the risk of surgery, ensure the safety of surgery.
