内镜下低温等离子腺样体消融联合鼓室置管术治疗儿童分泌性中耳炎的临床分析

摘　　要：目的探讨内镜下低温等离子腺样体消融术联合鼓室置管术治疗分泌性中耳炎的临床疗效。方法保守治疗无效的分泌性中耳炎80例(106耳)患儿分为2组,研究组采用内镜下低温等离子腺样体消融术联合鼓室置管术,观察组采用单纯鼓室置管术,观察对比两种治疗方法的治疗效果及采用COM-5评估量表评估术后生活质量改善情况。结果研究组患儿的临床有效率(100%)明显高于观察组(86.9%)(P<0.01),术后复发率明显低于后者(P<0.05),两组患者术后COM-5评分均较术前明显降低(P<0.01),其中研究组术后生活质量改善更为理想。结论内镜下低温等离子腺样体消融术联合鼓室置管术,疗效可靠,复发率低,能明显提高患儿术后生活质量,值得临床推广应用。Objective： To discuss the therapeutic effect of otitis media with effusion（ OME） by endoscopic adenoidectomy combined with tympanostomy tube in children. Method： 80 patients（ 106 ears） suffering from OME were randomly divided into 2 groups. The study group was treated with adenoidectomy combined with tympanostomy tube insertion under endouscope,and the control group was treated with simple Tympanostomy tube insertion. The effect of two treatments and COM-5 assessment scale were used to evaluate the improvement of quality of life. Result： The clinical efficiency of the children in the study group（ 100%） was significantly higher than that in the control group（ 86. 9%）（ P〈 0. 01）. The postoperative recurrence rate was significantly lower than that of the latter（ P〈 0. 05）. The COM-5 scores of the two groups were significantly lower than those before the operation（ P〈 0. 01）,and the improvement of the quality of life in the study group was more ideal. Conclusion： Endoscopic adenoidectomy combined with tympanostomy tube treantment appears to produce better clinical effection and lower postoperative recurrence rate in children with OME,and can improve quality of life. It is worth clinical application.
