分化型甲状腺癌首次^(131)Ⅰ治疗对唾液腺功能的辐射影响

摘　　要：目的探讨分化型甲状腺癌(DTC)首次^(131)Ⅰ治疗对唾液腺排泌功能的辐射影响。方法选择DTC手术后1~2个月首次行^(131)Ⅰ治疗的患者45例,^(131)Ⅰ治疗前当天行99mTc O-4唾液腺显像,1个月后再次行唾液腺显像。分析前后两次显像的结果,比较唾液腺排泌分数(SR)值的差异。结果 45例患者唾液腺功能异常率^(131)Ⅰ治疗前后两次显像分别为62.2%(28/45)、40.0%(18/45)。SR均值分别为:左腮腺(39.9%±2.5%)、(46.4%±1.2%),右腮腺(39.3%±2.0%)、(46.1%±0.8%),左颌下腺(29.7%±1.7%)、(36.6%±0.9%),右颌下腺(30.4%±1.9%)、(35.7%±0.9%)。t检验,P值均<0.05。结论首次^(131)Ⅰ治疗对唾液腺功能无明显影响。^(131)Ⅰ治疗后SR均数较治疗前有上升,可能与^(131)Ⅰ治疗过程中采取预防措施有关。Objective： To explore the radiation influence of salivary gland excretory function for first（131）Ⅰ therapy in patients with differentiated thyroid carcinoma（ DTC）. Method Choose 45 patients with DTC for the first time in（131）Ⅰ therapy after surgery operation 1 2 months. Before（131）Ⅰ therapy,they are all done99 mTc O-4 salivary glands scintigraphy in the therapy day. After（131）Ⅰ therapy to court review of a month,again salivary glands scintigraphy. Analyzing the results of the two scintigraphy before and after（131）Ⅰ therapy,comparing the difference between the salivary gland excretory function. Results The disfunction rate of salivary glands excretory function are 62. 2%（ 28/45） 、40. 0%（ 18/45） in the 45 scintigraphy salivary gland before and after（131）Ⅰ therapy. The twice scintigraphy salivary gland SR in 45 patients before（131）Ⅰ therapy for the first time and the treatment a month after mean respectively： the left parotid gland（ 39. 9% ± 2. 5%） 、（ 46. 4% ±1. 2%）,right parotid gland（ 39. 3% ± 2. 0%） 、（ 46. 2% ± 0. 8%）,left submandibular gland（ 29. 7% ± 1. 7%） 、（ 36. 6%± 0. 9%）,right submandibular gland（ 30. 4% ± 1. 9%） 、（ 35. 7% ± 0. 9%）. Through the t test,P〈 0. 05,the difference is statistically significant. Conclusion For the first time in（131）Ⅰ therapy of salivary gland excretory function no effect. After（131）Ⅰ therapy SR mean compare with before treatment,which related to take preventive measures in the process of treatment.
