摘　　要：目的:探讨高血压患者血清同型半胱氨酸(Hcy)的水平与脉压、血压变异性、血管内皮功能损害的关系。方法:选取180例原发性高血压患者,依据血清Hcy水平将患者分为高、中、低Hcy组,比较3组脉压、血压变异性、血管内皮功能,并将这些指标与Hcy水平进行相关性分析。结果:高Hcy组、中Hcy组、低Hcy组患者的脉压、24h收缩压标准差(24hSSD)、24h舒张压标准差(24hDSD)在三组间均呈逐渐降低的趋势,肱动脉介导的充血性血管内径变化率(FMD)、裸臂指数(ABI)在组间呈逐渐升高的趋势,组间两两比较差异具有统计学意义(P均<0.05);原发性高血压患者血清Hcy水平与脉压、24hSSD、24hDSD水平呈显著的正相关(P均<0.05),与患者的FMD、ABI测定值呈负相关(P均<0.05)。结论:高Hcy水平的高血压患者脉压、血压变异性较高,血管内皮功能损伤更严重。Objective：To investigate the relationship between serum Hcy levels and pulse pressure,blood pressure variability,endothelial dysfunction in hypertensive patients. Methods：A total of 180 patients with essential hypertension in our hospital were divided into three groups ：high,middle,and low Hcy group,comparing pulse pressure,blood pressure variability and endothelial function of each group. Results：The values of pulse pressure,24 h SSD and 24 h DSD showed a gradual decrease trend among the three groups（high Hcy group,middle Hcy group and low Hcy group）with statistically significant difference（P〈 0.05）;FMD and ABI showed a gradually increasing trend among the groups,and there was a statistically significant difference between the groups（P〈 0.05）.There was a significant positive correlation between serum Hcy levels and pulse pressure,24 h SSD,24 h DSD levels（P〈 0.05）;while there was a negative correlation between serum Hcy levels and FMD,ABI（P〈 0.05）. Conclusions：Pulse pressure,blood pressure variability and endothelial dysfunction were higher in hypertensive patients with high Hcy levels.
