摘 要:目的探讨血管紧张素转换酶2(ACE2)基因多态性与糖尿病合并2型心肾综合征的相关性。方法福建地区汉族的糖尿病患者共406例,其中84例并发2型心肾综合征,按是否并发2型心肾综合征分为2型糖尿病(T2DM)+心肾综合征(CRS)组和T2DM组;100例体检健康者为对照组。应用DNA测序和酶切电泳法检测ACE2基因A9570G多态性,并结合超声心动图参数及生化指标进行分析。结果在男性,T2DM+CRS组G等位基因频率/G基因型分布显著高于对照组及T2DM组(P<0.05)。在女性,3组间等位基因频率/基因型构成差异无统计学意义(P>0.05)。在T2DM+CRS组内,根据不同基因型对患者心、肾功能进行亚组分析发现,男性G基因型患者肾小球滤过率显著低于A等位基因(P<0.05);女性则未观察到这种差异(P>0.05)。结论 ACE2基因A9750G多态性可能与男性糖尿病并发2型心肾综合征的遗传易感性相关,还可能与其肾功能不全严重程度有关。Objective To explore the association between the polymorphism of A9570 Gin angiotensin converting enzyme2 gene and diabetes mellitus complicated with Type 2 cardiorenal syndrome.Methods A total of 406 non-sibship T2 DM patients in Fujian province,84 cases with Type 2 cardiorenal syndrome.All the patients were divided into T2 DM +CRS and T2 DM groups according to if on set of Type 2 cardiorenal syndrome,Polymerase chain reaction and restriction fragment length polymorphism were used to observe polymorphisms of ACE2 gene A9570 Gand genotyping to analyze combining with clinical,ultrasoundcardiogram parameters and biochemical indicator.Results The G allele frequencies and G genotypes frequencies in T2 DM +CRS group were significantly higher than those of control group and T2 DM group in male(P〈0.05);There was no significant difference in allele frequencies and the construction of genetype frequencies among three groups in female(P〉0.05).Subset analysis based on different genetypes in T2 DM +CRS group indicated that the Glomerular filtration rate in patients with G genetype was significantly higher than that in patients with A genetype in male(P〈0.05),but this difference was not observed in female(P〉0.05).Conclusion The polymorphism of A9570 Gin ACE2 gene maybe associated with the genetic predisposition of T2 DM complicated with Type 2 cardiorenal syndrome in male,and it is also associated with the severity of renal insufficiency.
