摘 要:目的观察溃结宁膏穴位敷贴对溃疡性结肠炎(UC)(脾肾阳虚证)大鼠白介素-17(IL-17)及叉头状转录因子3(Foxp3)水平的影响。方法将40只SD大鼠随机分为正常对照组、溃疡性结肠炎(脾肾阳虚证)模型组(模型组)、溃结宁膏穴位敷贴治疗组(敷贴组)、柳氮磺吡啶治疗组(SASP组)。建立溃疡性结肠炎(脾肾阳虚证)大鼠模型,造模结束后治疗28 d,观察大鼠一般情况,进行疾病活动指数(DAI)评分、结肠黏膜损伤指数(CMDI)评分、组织学损伤指数(TDI)评分,酶联免疫吸附(ELISA)检测各组大鼠血清及结肠组织IL-17、Foxp3水平。结果与正常对照组大鼠比较,模型组大鼠体质量下降、饮水量减少、肛温降低,大鼠疾病活动指数评分、结肠黏膜损伤指数评分及结肠组织学损伤指数评分均显著升高,大鼠IL-17水平明显增高,Foxp3水平明显降低(P<0.01);与模型组大鼠相比较,敷贴组、SASP组大鼠体重增加、饮水量增加、肛温升高,大鼠疾病活动指数评分、结肠黏膜损伤指数评分及结肠组织学损伤指数评分明显降低,大鼠IL-17水平明显降低,Foxp3水平明显增高(P<0.01);敷贴组、SASP组之间差异无统计学意义(P>0.05)。结论溃结宁膏穴位敷贴可减轻UC肠道炎症反应,对肠道黏膜具有修复作用,其部分作用机制是通过上调抑炎因子Foxp3并下调促炎因子IL-17的水平,恢复Th17/Treg平衡来达到治疗UC的目的。Objective: To observe the effect of acupoint application with Kuijiening Plaster on the level of interleukin-17(IL-17) and forkhead transcription factor 3(Forkhead box,P3,Foxp3) in ulcerative colitis(UC) rats with spleen-kidney Yang deficiency. Methods: 40 SD rats were randomly divided into four groups: the control group,the model group,Kuijiening Plaster group(abbreviated application group),and Sulfasalazine treatment group(SASP group). Ulcerative colitis models of spleen-kidney Yang deficiency syndrome in rats were established. After that,the treatment lasted for 28 days. The general situation of rats,scores of the disease activity index(DAI),scores of the colon macroscopic damage index and scores of the tissue damage index were recorded. The levels of IL-17 and Foxp3 in the serum and colon tissues of each group were detected with enzyme linked immunosorbent assay(ELISA). Results: Compared with the normal control group,the body weight,the amount of drinking water and the rectal temperature of rats in the model group all decreased. The disease activity index scores,colonic mucosal injury index scores and colon histological injury index scores all increased significantly.The level of IL-17 in rats significantly increased,and Foxp3 level significantly decreased(P〈0.01). Compared with the model group,the body weight of rats in the application group and SASP group increased quickly;the amount of drinking water and the rectal temperature increased. The disease activity index scores,colonic mucosal injury index scores and colon histological injury index scores all decreased significantly. The level of IL-17 in rats significantly decreased;Foxp3 level significantly increased(P〈0.01). There was no significant difference between the application group and SASP group(P〉0.05).
